摘要
目的:探讨社区居家老年人健康相关生命质量及医养结合服务需求,为提高老年人健康水平、优化养老资源供给提供建议。方法:采用多阶段分层整群随机抽样的方法,于2021年9月至10月抽取苏州市和广州市485名60岁及以上社区居家老年人为研究对象,由经过专业培训的调查员面对面进行调查,使用欧洲五维健康量表(EQ-5D量表)测量老年人的健康相关生命质量,并对医养结合的服务需求进行分析。结果:社区居家老年人EQ-5D健康效用值为0.696(0.505,1.000)。在5维度中,存在困难比例依次为日常生活能力(57.3%)、行动能力(55.3%)、自我照顾能力(51.1%)、疼痛或不舒服(42.5%)、焦虑或抑郁(24.9%)。在医疗服务需求方面更注重临床治疗、医学康复、医学护理、急救救护;健康管理服务方面以健康指导、康复训练、高血压管理为主;生活照料方面助餐、助厕、助浴、助行的需求较多。结论:社区居家老年人的健康相关生命质量偏低,建议为不同需求的老年人提供分梯度的服务,逐步完善医养结合服务需求,全方位提高老年人健康水平。
Aim:To explore the health-related quality of life and the demand for integrated medical and nursing services for the elderly living at home in the community,and to provide recommendations for improving the health of the elderly and optimising the supply of elderly resources.Methods:A multistage stratified whole-group random sampling method was used to select 485 community-dwelling adults aged 60 years and older in Suzhou and Guangzhou from September to October 2021.A face-to-face survey was conducted by professionally trained investigators to measure the health-related quality of life of the elderly using the European five dimensions questionnaire( EQ-5D),and factors influencing the demand for integrated medical and nursing services were analyzed.Results:The health utility value for the elderly living at home was 0.696( 0.505,1.000).The proportion of difficulties in the 5 dimensions was in the order of ability to perform daily activities( 57.3%),mobility( 55.3%),self-care( 51.1%),pain or discomfort( 42.5%),and anxiety or depression( 24.9%).In terms of medical service needs,more emphasis was placed on clinical treatment,medical rehabilitation,medical care and first aid;in terms of health management services,health guidance,rehabilitation training and hypertension management were the main ones;in terms of daily care,more demand was placed on meal assistance,toilet assistance,bathing assistance and mobility assistance.Conclusion:The health-related quality of life of the elderly living at home in the community is low,and it is recommended that a graded service should be provided for the elderly with different needs,gradually improving the demand for integrated medical and nursing services and improving the health of the elderly in a holistic manner.
