摘要
目的观察化痰清肺汤与穴位敷贴联合左氧氟沙星治疗社区获得性肺炎的临床效果。方法回顾性分析2019年11月—2020年10月厦门市第五医院收治的社区获得性肺炎患者80例,根据治疗方案不同分为观察组49例和对照组31例。对照组采用左氧氟沙星治疗,观察组在对照组基础上联合化痰清肺汤、穴位敷贴治疗,2组均连续治疗4周。比较2组患者治疗总有效率,症状改善时间,治疗前后实验室指标变化及不良反应。结果观察组患者治疗总有效率为97.96%,高于对照组的80.65%(χ^(2)=5.125,P=0.012);观察组患者发热、咳嗽、胸片炎性改变、肺部啰音改善时间均短于对照组(P<0.01);治疗4周后,2组白细胞(WBC)计数、中性粒细胞、超敏C反应蛋白(hs⁃CRP)、降钙素原(PCT)水平均较治疗前降低,且观察组低于对照组(P均<0.01);2组免疫球蛋白IgA、IgG、IgM水平均较治疗前升高,且观察组高于对照组(P均<0.01);治疗期间观察组不良反应总发生率为4.08%,低于对照组的22.58%(χ^(2)=4.787,P=0.024)。结论化痰清肺汤与穴位敷贴联合左氧氟沙星治疗社区获得性肺炎效果显著,可有效减轻炎性反应,缩短治疗时间,减少不良反应的发生,值得临床推广应用。
Objective To observe the clinical effect of Huatanqingfei decoction and acupoint application combined with levofloxacin in the treatment of community acquired pneumonia.Methods A retrospective analysis of 80 patients with community acquired pneumonia who were admitted to the Fifth Hospital of Xiamen from November 2019 to October 2020 was divided into an observation group of 49 cases and a control group of 31 cases according to different treatment plans.The control group was treated with levofloxacin,and the observation group was treated with Huatanqingfei decoction and acupoint applica⁃tion on the basis of the control group.Both groups were treated for 4 consecutive weeks.The total effective rate of treatment,symptom improvement time,changes in laboratory indicators before and after treatment and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of patients in the observation group was 97.96%,which was higher than 80.65%in the control group(χ^(2)=5.125,P=0.012);The improvement time of fever,cough,chest X⁃ray inflammatory reaction and pulmonary rales in the observation group was shorter than that in the control group(P<0.01);After 4 weeks of treatment,the white blood cell(WBC)count,neutrophils,high⁃sensitivity C⁃reactive protein(hs⁃CRP)and procalcitonin(PCT)levels in both groups were lower than those before treatment,and the observation group was lower than the control group(all P<0.01);The levels of immunoglobulin IgA,IgG,and IgM in both groups were higher than those before treatment,and the observation group was higher than the control group(all P<0.01);During the treatment period,the total incidence of adverse reactions in the observation group was 4.08%,which was lower than 22.58%in the control group(χ^(2)=4.787,P=0.024).Conclusion Huatanqingfei decoction and acupoint application combined with levofloxacin in the treatment of community acquired pneumonia can effectively reduce inflammatory reaction,shorten the treat⁃ment time and reduce the
