摘要
目的研究抗生素联合阴道用乳杆菌对因不孕症需行体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF)术合并慢性子宫内膜炎(chronic endometritis,CE)患者的治疗效果。方法回顾性分析2019年1月—2021年6月于徐州市中心医院因不孕症需行IVF助孕合并CE的患者共137例,根据治疗方式分为单纯抗生素治疗组(66例)、抗生素联合阴道用乳杆菌治疗组(60例)、未行治疗组(11例),比较3组患者的一般情况(年龄、不孕年限、体质量指数、基础性激素水平)及治疗后第1周期胚胎移植的妊娠结局(子宫内膜厚度、移植胚胎数、移植优质胚胎率、囊胚移植率、临床妊娠率、着床率、早期流产率、异位妊娠率)。结果抗生素联合阴道用乳杆菌治疗可改善CE患者的临床妊娠率及着床率,但不影响子宫内膜厚度。3组患者的一般情况、移植胚胎数、移植优质胚胎率、囊胚移植率、早期流产率及异位妊娠率差异均无统计学意义。结论抗生素联合阴道用乳杆菌可改善CE患者的妊娠结局,疗效优于单独使用抗生素。
Objective To determine the effect of antibiotic therapy combined with live lactobacillus capsule for vaginal use on infertile patients with chronic endometritis(CE)for in vitro fertilization and embryo transfer(IVF).Methods A total of 137 infertile patients with CE who underwent IVF at Xuzhou Central Hospital from January 2019 to June 2021 were enrolled and their clinical data were retrospectively analyzed.According to their treatments,the patients were divided into three groups:an antibiotic therapy group(n=66),an antibiotic therapy combined with live lactobacillus capsule for vaginal use group(n=60),and an untreated group(n=11).The three groups were compared for their basic information(age,infertile years,body weight index,and basic hormones)and the pregnancy outcomes after the first cycle of embryo transfer(endometrial thickness,the number of embryos transferred,high-quality embryo transfer rate,blastocyst transfer rate,clinical pregnancy rate,implantation rate,early abortion rate,and ectopic pregnancy rate).Results Antibiotic therapy combined with live lactobacillus capsule for vaginal use remarkably improved clinical pregnancy rate,implantation rate,without effects on endometrial thickness.There were no statistical differences in general information,the number of embryos transferred,high-quality embryo transfer rate,blastocyst transfer rate,early abortion rate,and ectopic pregnancy rate among the three groups.Conclusions Antibiotic therapy combined with live lactobacillus capsule for vaginal use can improve the pregnancy outcome of CE patients,with better effectiveness than antibiotic therapy alone.
