摘要
目的探讨刺络放血联合桃红四物汤用于踝关节骨折术前消肿的疗效观察。方法选取2018年3月—2020年9月期间保定市第一中心医院骨伤科收治的踝关节骨折患者90例,按照随机数字表法分为治疗组和对照组,每组各45例。对照组予以抬高患肢、冷敷、静脉滴注七叶皂苷钠等常规治疗,并口服中药桃红四物汤。治疗组在对照组治疗基础上予以刺络放血治疗。两组患者治疗7 d后。观察比较两组患者治疗前及治疗第3天、第5天肿胀程度评分、肿胀值和疼痛视觉模拟评分(Visual analogue scale,VAS),比较两组患者消肿时间和消肿速度,并比较两组患者并发症发生率。结果治疗第3天、第5天两组患者肿胀程度评分、肿胀值、VAS评分较治疗前均降低,差异有统计学意义(P<0.01),且治疗组均低于对照组,差异有统计学意义(P<0.01)。治疗组消肿时间少于对照组,消肿速度快于对照组,差异有统计学意义(P<0.01)。治疗组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论刺络放血联合桃红四物汤可快速消肿、减轻疼痛,减少并发症,有利于减少踝关节骨折患者手术等待时间,应用于踝关节骨折术前消肿效果显著。
Objective To investigate the effect of bloodletting combined with Taohong Siwu Decoction on preoperative swelling of ankle fracture.Methods A total of 90 patients with ankle fracture who were treated in the Department of Orthopedics and Traumatology,Baoding No.1 Central Hospital from March 2018 to September 2020 were selected and randomized into the control group and observation group,with 45 in either group.The control group received routine treatment such as lifting the affected limb,cold compress,intravenous drip of sodium aescinate,and oral administration of Taohong Siwu Decoction.The observation group was treated by bloodletting in addition to the routine treatment in the control group.The treatment course of the two groups was 7 days.The swelling degree score,swelling value,and visual analogue scale(VAS)score of pain were compared between the two groups before treatment and on the 3^(rd) and 5^(th) day of treatment.The time and speed of detumescence and the incidence of complications were also compared between the two groups.Results The swelling degree score,swelling value,and VAS score of the two groups on 3^(rd) and 5^(th) day were lower than those before treatment(P<0.01)and the three indexes were lower in the observation group than in the control group(P<0.01).The detumescence in the observation group was earlier and faster than that in the control group(P<0.01).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Bloodletting combined with Taohong Siwu Decoction can rapidly alleviate swelling and pain and reduce complications,which can save the time of ankle fracture patients before operation.In summary,the combination has significant effect on preoperative swelling of ankle fracture.
