摘要
目的探讨血清β2-微球蛋白(β2-MG)联合C反应蛋白(CRP)与白蛋白比值(CAR)对首诊断多发性骨髓瘤预后的预测价值。方法回顾性选择2018年7月至2021年7月阜阳市人民医院收治的78例多发性骨髓瘤患者为研究对象。随访时间2~39个月,统计78例患者预后情况,根据患者的预后情况分为生存组与死亡组,收集患者的临床资料,以Logistic多因素回归分析首诊断多发性骨髓瘤患者死亡的危险因素,制作受试者工作特征曲线(ROC),以曲线下面积(AUC)评估血清β2-MG与CAR及二者联合对首诊断多发性骨髓瘤患者死亡的预测价值。结果截止随访日期2022年1月,中位随访时间17.19个月,78例患者死亡18例,死亡率为23.08%,剩余60例均生存。死亡组KPS评分低于生存组,差异有统计学意义(P<0.05);死亡组血清β2-MG与CAR水平高于生存组,差异有统计学意义(P<0.05)。Logistic多因素回归分析结果显示,KPS评分、ISS分期为Ⅲ期、血清β2-MG与CAR水平均为首诊断多发性骨髓瘤患者死亡的危险因素(P<0.05);ROC曲线结果显示,血清β2-MG与CAR及二者联合预测首诊断多发性骨髓瘤患者死亡的AUC值(95%CI)分别为0.841(0.740~0.914)、0.728(0.615~0.822)、0.879(0.786~0.942)(P<0.05)。结论血清β2-MG与CAR在预测首诊断多发性骨髓瘤患者预后的预测中具有重要价值,且二者联合的预测价值更高。
Objective To investigate the predictive value of serumβ2-microglobulin(β2-MG)combined with C-reactive protein(CRP)to albumin ratio(CAR)in the prognosis of first-diagnosed multiple myeloma.Methods A total of 78 patients with first-diagnosed multiple myeloma who were admitted to the Fuyang People’s Hospital from July 2018 to July 2021 were selected as the research objects.Follow-up time from 2 to 39 months,the prognosis of 78 patients were counted.According to the prognosis of patients,they were divided into survival group and death group.Clinical data of patients were collected.Multivariate logistic regression was used to analyze risk factors for death in patients with first-diagnosed multiple myeloma receiver operating characteristic curve(ROC)was prepared,and the area under the curve(AUC)was used to evaluate the predictive value of serumβ2-MG and CAR and their combination in the death of patients with first-diagnosed multiple myeloma.Results The cut-off date of follow-up was January 2022,and the median follow-up time was 17.19 months,18 of 78 patients died,the mortality rate was 23.08%,and the remaining 60 patients survived.The KPS score of the death group was lower than that of the survival group(P<0.05).The death group serumβ2-MG and CAR levels than the survival group(P<0.05).Logistic multivariate regression analysis showed that KPS score,ISS stageⅢ,serumβ2-MG and CAR levels were risk factors for death in patients with first-diagnosed multiple myeloma(P<0.05).The ROC curve results showed that the AUC values(95%CI)of serumβ2-MG and CAR and their combination in predicting the death of the first-diagnosed multiple myeloma patients were 0.841(0.740-0.914),0.728(0.615-0.822),0.879(0.786~0.942)(P<0.05).Conclusion Serumβ2-MG and CAR have important value in predicting the prognosis of first-diagnosed multiple myeloma patients,and the combination of the two has a higher predictive value.
