摘要
目的探讨多发性骨髓瘤(MM)患者血清白细胞介素-37(IL-37)、血管内皮生长因子(VEGF)、乳酸脱氢酶(LDH)和β2微球蛋白(β2-MG)水平变化及其临床意义。方法选取2015年1月至2019年7月该院收治的初诊MM患者62例(MM组)和健康体检者28例(对照组),采用ELISA法检测血清IL-37、VEGF水平,免疫比浊法测定血清LDH、β2-MG水平,比较两组及国际分期体系(ISS)不同分期、不同疗效MM患者各指标水平,分析血清IL-37和β2-MG的相关性。结果MM组患者血清IL-37水平明显低于对照组,VEGF、LDH、β2-MG水平明显高于对照组(P<0.05);不同ISS分期MM患者血清IL-37、VEGF、LDH、β2-MG水平存在明显差异(P<0.05),且Ⅲ期MM患者IL-37水平低于Ⅰ期和Ⅱ期患者,VEGF、LDH、β2-MG水平高于Ⅰ、Ⅱ期患者(P<0.05);与化疗前比较,化疗后MM患者血清IL-37水平升高,VEGF、LDH、β2-MG水平降低(P<0.05);化疗后评估疗效,完全缓解MM患者血清IL-37水平高于部分缓解(PR)、疾病进展(PD)患者,而VEGF、LDH、β2-MG水平低于PR、PD患者(P<0.05);MM患者血清IL-37与β2-MG水平呈负相关(R2=0.737,P<0.05)。结论MM患者血清IL-37、VEGF、LDH和β2-MG水平变化与肿瘤负荷、疾病分期、近期疗效相关,可能为临床诊疗提供有利的实验室依据。
Objective To investigate the changes and clinical significances of serum interleukin-37(IL-37),vascular endothelial growth factor(VEGF),lactate dehydrogenase(LDH)andβ2-micro-globulin(β2-MG)in patients with multiple myeloma(MM).Methods From January 2015 to July 2019,62 MM patients admitted to the hospital for initial diagnosis(the MM group)and 28 healthy physical examination people(the control group)were selected.The levels of serum IL-37 and VEGF were detected by ELISA.LDH andβ2-MG levels were determined by immunoturbidimetry.The indexes of MM patients of the two groups,with different international staging system(ISS)stage and different curative effects were compared.And the correlation between serum IL-37 andβ2-MG was compared.Results The level of serum IL-37 in the MM group was significantly lower than that in the control group,and the levels of VEGF,LDH andβ2-MG were significantly higher(P<0.05).The levels of IL-37,VEGF,LDH andβ2-MG in MM patients with different ISS stages were significantly different(P<0.05).The level of IL-37 in MM patients at stageⅢwas lower than that at stageⅠandⅡ,while the levels of VEGF,LDH andβ2-MG were higher(P<0.05).Compared with before chemotherapy,serum IL-37 level in MM patients was higher after chemotherapy,and the levels of VEGF,LDH,andβ2-MG were lower(P<0.05).As the efficacy after chemotherapy,the serum IL-37 level of patients with complete remission of MM was higher than that of patients with partial remission(PR)and disease progression(PD),while the levels of VEGF,LDH,andβ2-MG were lower(P<0.05).Serum IL-37 andβ2-MG levels were negativelycorrelated in MM patients(R2=0.737,P<0.05).Conclusion The changes in serum IL-37,VEGF,LDH andβ2-MG levels in patients with MM are related to tumor burden,disease stage,and short-term efficacy,which may provide a favorable experimental basis for clinical diagnosis and treatment.
