摘要
目的观察补肾活血方治疗肾虚血瘀型多发性骨髓瘤的临床疗效。方法随机选择2018年6月—2019年12月在该院血液科治疗的多发骨髓瘤患者80例进行临床研究。使用随机数字法将患者分为观察组和对照组,每组各40例。对照组患者给予VTD方案(硼替佐米+沙利度胺+地塞米松),观察组在对照组的基础上加用补肾活血方。21 d为1个疗程,共治疗2个疗程。比较两组患者治疗前和治疗2个疗程后临床疗效、中医证候评分、临床指标以及生存质量的变化。结果治疗2个疗程后,观察组治疗总有效率为87.50%(35/40),明显高于对照组67.50%(27/40)(P<0.05);观察组中医证候评分低于对照组(P<0.05);观察组M蛋白、骨髓浆细胞计数、尿素氮均低于对照组,血红蛋白高于对照组(P<0.05);QLQ-C30评分方面,观察组整体生活质量、躯体功能、情绪功能评分高于对照组,疲乏、恶心呕吐、睡眠障碍、食欲减退、便秘、腹泻、疼痛低于对照组(P<0.05)。结论补肾活血方治疗肾虚血瘀型多发性骨髓瘤临床疗效良好,能够提高患者的生存质量,具有重要的临床应用价值。
Objective To observe the clinical effect of Invigorating Kidney and Promoting Blood Circulation Decoction in the treatment of multiple myeloma(MM)with kidney deficiency and blood stasis syndrome.Methods Eighty patients with MM were collected in the wards of the Hematology Department of Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from June 2018 to December 2019.They were randomly divided into treatment group and control group,with 40 cases in each group.Both groups were treated with VTD chemotherapy regimens(bortezomib,thalidomide,dexamethasone)while the treatment group was also treated with Invigorating Kidney and Promoting Blood Circulation Decoction.After two treatment courses,we recorded the changes of clinical effect,TCM syndrome grade,laboratory indexes and QLQ-C30.Results The total effective rates of the treatment group and the control group were 87.50%(35/40)and 67.50%(27/40)after two treatment courses(P<0.05).TCM syndrome grades in the treatment group were lower than those in the control group(P<0.05).The degree of M protein,bone marrow plasmacyte and blood urea nitrogen in the treatment group were lower.The degree of hemoglobin in the treatment group was higher than that in the control group(P<0.05).From the perspective of QLQ-C30 questionnaire,the scores of global quality of life,physical functioning and emotional functioning in the treatment group were higher than those in the control group,and the scores of fatigue,nausea and vomiting,dyssomnia,anorexia,constipation,diarrhea and pain were lower than those in the control group(P<0.05).Conclusion Invigorating Kidney and Promoting Blood Circulation Decoction is effective in treating MM with kidney deficiency and blood stasis syndrome,improving the quality of life of patients and has great value in clinical application.
