摘要
目的探讨不同类型妊娠期高血压疾病中同型半胱氨酸(homocysteine,Hcy)、叶酸及维生素B_(12)(vitamin B_(12),VitB_(12))的水平差异。方法选取2018年7月-2021年1月潍坊医学院附属医院就诊的140例妊娠期高血压患者为疾病组,分为妊娠期高血压组50例,子痫前期组50例,重度子痫前期组40例,并将同期本院进行产检的50名健康孕妇为健康组。于确诊后治疗前检测Hcy、叶酸、VitB_(12)的水平。结果疾病组Hcy水平显著高于健康组、叶酸水平低于健康组,差异有统计学意义(P<0.05),疾病组和健康组VitB_(12)水平差异无统计学意义(P>0.05)。重度子痫前期组Hcy水平,依次高于子痫前期组、妊娠期高血压组和健康组,差异有统计学意义(P<0.05);重度子痫前期组叶酸水平,依次低于子痫前期组、妊娠期高血压组和健康组,差异有统计学意义(P<0.05);4组VitB_(12)水平比较,差异无统计学意义(P>0.05)。Hcy水平与妊娠期高血压疾病严重程度呈正相关(r=0.473,P<0.05)。叶酸水平与疾病严重程度呈负相关(r=-0.576,P<0.05),VitB_(12)与疾病严重程度无相关性(r=-0._(12)0,P>0.05)。结论监测妊娠期间Hcy水平,并适当补充叶酸对预防妊娠期高血压疾病的发生,预测病情的发展存在积极作用。
Objective To investigate the differences in the levels of homocysteine(Hcy),folic acid and vitamin B_(12)(VitB_(12)) in different types of gestational hypertension.Methods A total of 140 patients with gestational hypertension who were treated in the Affiliated Hospital of Weifang Medical College from July 2018 to January 2021 were selected as the research objects(disease group).Divided into gestational hypertension group 50 cases,preeclampsia group 50 cases,severe preeclampsia group 40 cases.50 healthy pregnant women who underwent obstetric examination in the hospital during the same period were selected as the healthy group.The levels of Hcy,folic acid and VitB_(12) were de⁃tected after diagnosis and before treatment.Results The level of Hcy in the disease group was significantly higher than that in the healthy group,and the level of folic acid was lower than that in the healthy group,and the difference was statistically significant(P<0.05).The level of Hcy in the severe preeclampsia group was higher than the pre⁃eclampsia group,the gestational hypertension group and the healthy group in turn,and the difference was statistically significant(P<0.05).The level of folic acid in the severe preeclampsia group was lower than the preeclampsia group,the gestational hypertension group and the healthy group in turn,and the difference was statistically significant(P<0.05).Comparison of VitB_(12)levels in the 4 groups,the difference was not statistically significant(P>0.05).The level of Hcy was positively correlated with the severity of gestational hypertension(r=0.473,P<0.05).The level of folic acid was negatively correlated with disease severity(r=-0.576,P<0.05),the VitB_(12)was not correlated with the disease se⁃verityt(r=-0._(12)0,P>0.05).Conclusion Monitoring Hcy level during pregnancy and appropriate supplementation of fo⁃lic acid have positive effects on preventing the occurrence of gestational hypertension and predicting the development of the disease.
