自体月经血来源的干细胞在宫腔粘连中的临床应用

摘要 目的探究自体月经血来源的干细胞在宫腔粘连中的临床应用。方法选取2018年4月—2021年4月于桂平市人民医院就诊的50例不孕患者,其均在宫腔镜中被诊断为中、重度宫腔粘连,观察自体月经血干细胞移植后月经的改变、子宫内膜厚度及形态、妊娠结局情况。结果50例患者的经期、经量均较治疗前增加;子宫内膜厚度较治疗前增厚,差异有统计学意义(t=7.89、14.49、31.27,P<0.001);子宫内膜形态较治疗前明显好转,均为A型或B型(50 vs 13),差异有统计学意义(χ^(2)=59.33,P<0.001),其中有23例临床妊娠(妊娠率为46%),3例双胎妊娠,其余为单胎妊娠。结论自体月经血来源的干细胞在一定程度上能使受损的子宫内膜再生并恢复功能,改善宫腔粘连的症状。 Objective To investigate the clinical application of autologous menstrual blood-derived stem cells in intrauterine adhesions.Methods 50 infertile patients who were treated in Guiping People’s Hospital from April 2018 to April 2021,all of whom were diagnosed as moderate or severe intrauterine adhesions by hysteroscopy,were used to observe the changes in menstruation after autologous menstrual blood stem cell transplantation,endometrial thickness and shape,pregnancy outcome.Results The menstrual period and menstrual volume of the 50 patients were increased compared with those before treatment.Endometrial thickness was thicker than before treatment,and the difference was statistically significant(t=7.89,14.49,31.27,P<0.001).The endometrial morphology was significantly improved compared with before treatment,all were type A or type B(50 vs 13),and the difference was statistically significant(χ^(2)=59.33,P<0.001).There were 23 clinical pregnancies(pregnancy rate 46%),3 twin pregnancies and the rest singleton pregnancies.Conclusion Stem cells derived from autologous menstrual blood can regenerate and restore the damaged endometrium to a certain extent,improve the symptoms of intrauterine adhesions.
