摘要
阻塞性睡眠呼吸暂停低通气综合征(OSAHS)是一种临床常见的睡眠呼吸障碍性疾病,影响到从儿童到老人的全体人群,是导致多器官损伤、衰竭,甚至死亡的重要原因,且发病率呈不断增长趋势。甲状腺功能减退症也是临床常见疾病,同样可以导致多系统器官功能损害,并加重其他合并疾病的病情。当前的研究显示,甲状腺功能减退症是引起OSAHS的危险因素之一,可加重OSAHS的病情进展。同时,OSAHS的间歇低氧等病理生理损害也会影响到甲状腺的结构和功能,导致在OSAHS患者中甲状腺功能减退症(尤其是亚临床甲状腺功能减退症)的发生率较高。这两种疾病相互影响的关系和详细机制尚未得到很好的描述。两种疾病的交叉重叠,不仅会使病情更为严重复杂,而且部分临床表现的相似性还会造成某一疾病对另一个疾病的掩盖,导致漏诊和治疗不当。因此,加强医务人员对这两类疾病关系的认识,能很大程度上减少临床中的误诊、漏诊以及病情的延误。本文将对OSAHS与甲状腺功能减退症之间的流行病学、相关关系、治疗等方面展开综述。
Obstructive sleep apnea-hypopnea syndrome(OSAHS),a common clinical sleep-related respiratory disorder condition impacting the overall population from children to the elderly,is an important cause for multiple organ dysfunction&failure and even death of population with continual growth of incidence.Hypothyroidism,another a common clinical disease,can also lead to multi-system&organ dysfunction,and aggravates the condition of other combined diseases.The present studies show that hypothyroidism is one of risk factors leading to OSAHS and may aggravate the progress of OSAHS.Moreover,intermittent hypoxia and other pathophysiological damage of OSAHS will also impair the structure and functions of thyroid,and cause higher incidence of hypothyroidism(especially subclinical hypothyroidism)of OSAHS patients,while the relations between two diseases and their detailed mechanisms remain being unclearly described.The overlapping of two diseases will result in more serious and complicated disease,and the similarity of some clinical features will cause one disease to cover up another disease,leading to missed diagnosis and improper treatment.Finally,the intensification of medical workers'understandings on the relations between such two diseases can significantly decrease the misdiagnosis,missed diagnosis and delay in the clinical conditions.This paper will review the relations between OSAHS and hypothyroidism with respect to the epidemiology,correlation and treatment.
