椎体成形术与椎体后凸成形术对骨质疏松性椎体压缩性骨折患者骨代谢及腰椎功能的影响比较 被引量:4

摘要 目的:对比椎体成形术与椎体后凸成形术对骨质疏松性椎体压缩性骨折患者骨代谢及腰椎功能的影响。方法:选择2019年5月—2020年6月广东医科大学附属第二医院骨科收治的210例骨质疏松性椎体压缩性骨折患者作为研究对象,根据随机数表法分为椎体成形术组和椎体后凸成形术组,每组各105例。椎体成形术组患者采用椎体成形术治疗,椎体后凸成形术组患者采用椎体后凸成形术治疗,两组患者均治疗3个月。比较两组患者手术指标,治疗前后骨代谢生化指标、影像学指标及腰椎功能。结果:椎体成形术组术中透视次数、治疗费用、手术时间低于椎体后凸成形术组,差异有统计学意义(t=20.001、33.350、19.767,P<0.05),手术切口长度、骨水泥用量组间比较,差异无统计学意义(t=0.557、0.659,P>0.05)。治疗后,两组患者血清N端骨钙素(N-MID)、血清25羟锥维生素D(25-OH-D)水平较治疗前均升高,椎体后凸成形术组高于椎体成形术组,差异有统计学意义(t=10.599、7.132,P<0.05),血清β-胶原降解产物(β-CTX)水平较治疗前降低,椎体后凸成形术组低于椎体成形术组,差异有统计学意义(t=10.003,P<0.05)。治疗后,两组患者椎体前缘高度丢失率、Cobb角、ODI得分均较治疗前显著降低,差异有统计学意义。结论:椎体成形术与椎体后凸成形术治疗骨质疏松性椎体压缩性骨折,均可改善患者腰椎功能及影像学指标,椎体成形术手术指标优于椎体后凸成形术,但椎体后凸成形术可一定程度改善患者骨代谢。 Objective:To compare the effects of vertebroplasty and kyphoplasty on bone metabolism and lumbar function in pa⁃tients with osteoporotic vertebral compression fractures.Methods:A total of 210 patients with osteoporotic vertebral body compres⁃sion fractures admitted to the hospital from May 2019 to June 2020 were selected as the research subjects,and were divided into vertebroplasty group and kyphoplasty group according to the random number table method,with 105 cases in each group.The pa⁃tients in the vertebroplasty group were treated with vertebroplasty,and the patients in the kyphoplasty group were treated with ky⁃phoplasty.Both groups were treated for 3 months.The surgical indexes,biochemical indexes of bone metabolism,imaging indexes and lumbar spine function before and after treatment were compared between the two groups.Results:The number of intraopera⁃tive fluoroscopy,treatment cost,and operation time in the kyphoplasty group were lower than those in the kyphoplasty group,and the differences were statistically significant(t=20.001,33.350,19.767,P<0.05).There was no statistically significant difference in the length of surgical incision and the amount of bone cement between the groups(t=0.557,0.659,P>0.05).After treatment,the levels of serum N-MID and 25-OH-D in the two groups were increased compared with those before treatment,and the kyphoplasty group was higher than the vertebroplasty group,and the difference was statistically significant(t=10.599,7.132,P<0.05).The se⁃rumβ-CTX level was lower than that before treatment,and the kyphoplasty group was lower than the vertebroplasty group,and the difference was statistically significant(t=10.003,P<0.05).After treatment,the loss rate of anterior vertebral body height,Cobb an⁃gle and ODI score in the two groups were significantly lower than those before treatment.Conclusion:Both vertebroplasty and ky⁃phoplasty in the treatment of osteoporotic vertebral compression fractures can improve lumbar spine function and imaging indica⁃tors.Kyphoplasty
