摘要
目的观察清利活血解毒方联合西药用于老年急性痛风性关节炎的临床疗效。方法选取126例老年急性痛风性关节炎患者,以随机数表法分为观察组与对照组,各63例。对照组口服双氯芬酸钠缓释片、碳酸氢钠片治疗,观察组在对照组基础上加用清利活血解毒方治疗,2组均治疗3 d后评估治疗效果。比较2组治疗后临床疗效差异,比较2组治疗前后关节症状、体征(关节疼痛、关节肿胀、关节压痛、关节功能)评分及血清[C反应蛋白(CRP)、血沉(ESR)、肌酐(Cr)、血尿酸(SUA)]水平差异,统计并比较2组用药期间的不良反应发生率。结果观察组临床疗效(84.13%,53/63)优于对照组(66.67%,42/63)(P<0.05)。观察组治疗后各项评分优于治疗前及对照组治疗后(P<0.05)。观察组治疗后各项实验室指标优于治疗前及对照组治疗后(P<0.05)。2组用药期间不良反应总发生率比较,无显著差异(P>0.05)。结论清利活血解毒方联合西药治疗老年急性痛风性关节炎效果显著,能降低血清TCRP、ESR、Cr、SUA水平,且安全性良好。
Objective To observe the clinical effect of Qingli Huoxue Jiedu recipe combined with western medicines on acute gouty arthritis in the elderly.Methods A total of 126 elderly patients with acute gouty arthritis were selected and divided into an observation group and a control group by the random number table method,with 63 cases in each group.The control group was treated with diclofenac sodium sustained-release tablets and sodium bicarbonate tablets orally,while the observation group was additionally treated with Qingli Huoxue Jiedu recipe on the basis of the control group.The treatment effect of both groups was evaluated after 3 days of treatment.The difference in clinical efficacy between the two groups were compared after treatment,and the scores of joint symptoms and signs(joint pain,joint swelling,joint tenderness,and joint function)and the levels of serums[C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),creatinine(Cr)and serum uric acid(SUA)]before and after treatment were compared,and the incidence of adverse reactions during the treatment of the two groups was counted and compared.Results The clinical efficacy of the observation group(84.13%,53/63)was better than that of the control group(66.67%,42/63)(P<0.05).The scores of the observation group after treatment were better than those before treatment and those in the control group after treatment(P<0.05).The laboratory indexes in the observation group after treatment were better than those before treatment and those in the control group after treatment(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups during the treatment(P>0.05).Conclusion Qingli Huoxue Jiedu recipe combined with western medicines has a significant effect in the treatment of acute gouty arthritis in the elderly,which can reduce the levels of serum TCRP,ESR,Cr and SUA,with a good safety.
