MDT-CBL教学法结合标准化患者在肿瘤内科住院医师规范化培训临床教学中的应用研究 被引量:6

摘要 目的 评估以多学科协作治疗(MDT)-病例为基础(CBL)教学法与标准化患者(SP)相结合的新型教学方式在肿瘤内科住院医师规范化培训中的临床教学效果,并积极探寻新的教学方法。方法 选择2019年1月-2021年12月在某医院肿瘤内科进行住院医师规范化培训的80名医师作为研究对象,随机分为对照组和实验组两组,每组各40名。对照组采用传统教学法进行教学,实验组在MDT-CBL教学法的基础上引入SP,比较两组住院医师规范化培训医师的出科考核成绩及对教学效果的评价情况。结果 实验组住院医师规范化培训医师的出科考核理论成绩、技能成绩和病例分析成绩均显著高于对照组,差异具有统计学意义(P<0.05);此外,实验组在学习主动性、利于知识掌握、利于临床思维培养、利于医患沟通能力培养及教师授课的满意率方面均高于对照组,差异具有统计学意义(P<0.05);而两组在学习效率和学习兴趣方面均无统计学差异(P>0.05)。结论 在肿瘤内科住院医师规范化培训的临床教学中,应用MDT-CBL教学法与SP相结合的新型教学方式,能够帮助他们显著提高肿瘤内科专业知识水平、解决临床实际问题的能力,同时有助于临床诊疗思维和医患沟通能力的培养,并显著提升临床教学效果,值得在今后的临床教学中借鉴和进一步推广。 Objective To evaluate the effectiveness of Multiple Disciplinary Team(MDT)-Case Based Learning(CBL) teaching method combined with Standardized Patient(SP) in clinical teaching of standardized training for medical oncology residents, and to actively explore new teaching methods.Methods Eighty residents receiving training standardized in the department of medical oncology of a hospital from January 2019 to December 2021 were enrolled as the study objects, and they were randomly divided into control group and experimental group, with 40 subjects in each group. The control group received traditional teaching method while the MDT-CBL teaching method combined with SP was applied in the experimental group. Then compared the examination results of two groups and evaluate the teaching method.Results The scores of theoretical test, skill test results and case analysis of the experimental group was significantly higher than the control group, and the differences were statistically significant(P<0.05). In addition, the scores of the aspects of learning initiative, knowledge acquisition, clinical thinking training, doctor-patient communication ability and satisfaction on training physicians in the experimental group were higher than those of the control group were statistically significant(P<0.05). Whereas no difference was observed in learning efficiency and learning interest between the two groups(P>0.05).Conclusions The combination of MDT-CBL teaching method and SP applied in clinical teaching of standardized training for medical oncology residents could significantly improve their professional knowledge and ability to solve practical clinical problems, meanwhile, it could also help cultivate the training of clinical diagnosis-treatment thinking and doctor-patient communication skills, as well as significantly improve the teaching effect of teachers, which is worthy of reference and further promotion.
