心外膜脂肪组织CT衰减与冠状动脉狭窄程度的相关性研究 被引量:7

Correlation between CT Attenuation of Epicardial Adipose Tissue and Degree of Coronary Artery Stenosis

摘要 目的探讨冠心病患者心外膜脂肪CT衰减与冠状动脉狭窄程度的相关性。方法回顾性搜集经冠状动脉CT血管成像(CCTA)发现有冠状动脉狭窄的住院患者172例,根据血管病变最严重部位作为判定标准(轻度狭窄<50%,50%≤中度狭窄<75%,重度狭窄≥75%)将患者分为轻度狭窄组、中度狭窄组和重度狭窄组。分析并记录患者基本资料,根据CCTA结果评价患者冠状动脉Gensini积分、心外膜脂肪组织体积(EAT volume)、心外膜脂肪组织CT衰减(EAT CT attenuation),初步比较EAT volume、EAT CT attenuation和冠状动脉狭窄程度的关系,进一步分析冠心病患者EAT volume、EAT CT attenuation与冠状动脉狭窄程度的相关性,最后利用受试者工作特征(ROC)曲线评估EAT volume、EAT CT attenuation对冠状动脉重度狭窄预测的诊断效能。结果冠状动脉不同狭窄组比较发现年龄、Gensini积分和EAT volume、EAT CT attenuation差异均有统计学意义;对EAT volume、EAT CT attenuation和冠状动脉Gensini积分作Pearson相关性分析发现,EAT volume和Gensini积分呈正相关(r=0.338,P<0.001),EAT CT attenuation和Gensini积分呈负相关(r=-0.506,P<0.001)。对EAT volume、EAT CT attenuation做ROC曲线分析发现EAT volume+EAT CT attenuation二者联合对冠状动脉重度狭窄的预测价值更高,曲线下面积0.810,敏感性81.8%,特异性64.7%。结论EAT volume和EAT CT attenuation均能反映冠状动脉狭窄程度并对冠状动脉重度狭窄具有一定的预测价值,两者联合预测更有意义。 Objective To investigate the correlation between CT attenuation of epicardial adipose tissue and the degree of coronary artery stenosis in patients with coronary heart disease.Methods A retrospective study was conducted on 172 inpatients with coronary stenosis found by CCTA in the Affiliated Hospital of Xuzhou Medical University from January 2020 to January 2021.The criteria were based on the site of the most severe vascular lesion(mild stenosis<50%,50%≤moderate stenosis<75%,severe stenosis≥75%),which were divided into mild stenosis group,moderate stenosis group,and severe stenosis group.The basic data of patients were analyzed and recorded.In addition,the coronary Ginsini score,EAT volume and EAT CT attenuation were evaluated according to the CCTA results.Compared the relationship between EAT volume and EAT CT attenuation and the degree of coronary artery stenosis among mild,moderate and severe stenosis groups,and further analyze the correlation between EAT volume and EAT CT attenuation and the degree of coronary artery stenosis among patients with coronary heart disease.Finally,receiver operating characteristic curve(ROC)was used to evaluate the diagnostic efficacy of EAT volume and EAT CT attenuation in predicting severe coronary artery stenosis.Results Comparison of the three coronary artery stenosis groups showed statistically significant differences in age,Gensini score,EAT volume and EAT CT attenuation.By Pearson correlation analysis of EAT volume,EAT CT and Ginsini integration,it was found that there was positive correlation between EAT volume and Ginsini integration(r=0.338,P<0.001;r=0.506,P<0.001),and there was negative correlation between EAT CT attenuation and Ginsini integration(r=-0.506,P<0.001).The ROC curve analysis of EAT volume and EAT CT attenuation found that the prediction value of EAT volume+EAT CT combination in severe coronary artery stenosis was the highest,the area under the curve was 0.810,the sensitivity and specificity were 81.8%and 64.7%.Conclusion EAT volume and EAT CT attenuati
