血清胆红素与尿酸检测在冠心病临床诊断中的意义探析 被引量:6

摘要 目的探讨血清胆红素和尿酸检测在冠心病诊断中的意义。方法选择53例确诊冠心病患者作为观察组,另选择同期53例健康者作为对照组。两组均进行血清胆红素及尿酸检测。比较两组血清胆红素及尿酸水平,观察组不同冠状动脉狭窄程度积分法(Gensini)积分患者血清胆红素和尿酸水平。结果观察组患者血清间接胆红素(6.93±0.78)μmol/L、直接胆红素(4.82±0.35)μmol/L、总胆红素(11.75±1.87)μmol/L均低于对照组的(9.76±1.07)、(6.43±0.55)、(16.19±1.67)μmol/L,且尿酸(381.35±12.84)μmol/L高于对照组的(269.75±13.65)μmol/L,差异具有统计学意义(P<0.05)。观察组患者的血清间接胆红素、直接胆红素、总胆红素水平随着Gensini积分升高而降低,尿酸水平随着Gensini积分升高而升高,差异具有统计学意义(P<0.05)。结论冠心病与血清胆红素水平降低和尿酸水平升高有密不可分的关系,可以通过动态检测患者尿酸和血清胆红素水平来提升冠心病的诊断准确性。 Objective To discuss the significance of serum bilirubin and uric acid in clinical diagnosis of coronary heart disease.Methods 53 patients with confirmed coronary artery disease were selected as the observation group,and another 53 healthy subjects in the same period were selected as the control group.Serum bilirubin and uric acid tests were performed in both groups.Comparison was made on serum bilirubin and uric acid levels between the two groups,and the serum bilirubin and uric acid levels in patients with different coronary artery stenosis(Gensini)scores of the observation group.Results The serum indirect bilirubin(6.93±0.78)μmol/L,direct bilirubin(4.82±0.35)μmol/L and total bilirubin(11.75±1.87)μmol/L in the observation group were lower than(9.76±1.07),(6.43±0.55),and(16.19±1.67)μmol/L in the control group,and the uric acid(381.35±12.84)μmol/L was higher than(269.75±13.65)μmol/L in the control group.All the differences were statistically significant(P<0.05).In the observation group,the serum indirect bilirubin,direct bilirubin and total bilirubin decreased with the increase of Gensini score,and the uric acid increased with the increase of Gensini score,and the difference was statistically significant(P<0.05).Conclusion The decrease of serum bilirubin level and the increase of uric acid level are closely related to coronary heart disease.The diagnostic accuracy of coronary heart disease can be improved by dynamically detecting the levels of uric acid and serum bilirubin in patients.
