高血压冠状动脉病变与心外膜脂肪组织的相关性研究

摘要 目的分析心外膜脂肪组织(epicardial adipose tissue,EAT)与高血压患者冠状动脉病变的相关性。方法选取2019年1月—2021年12月新乡医学院第一附属医院高血压合并冠状动脉病变患者102例,均行冠状动脉造影检查和经胸多普勒超声检查,按照冠脉造影评分(Gensini)分为试验组(Gensini评分≥40分)45例和对照组(Gensini评分<40分)57例,对比两组EAT厚度、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、总胆固醇(TC)、超敏C反应蛋白(hs-CRP)、胰岛素抵抗指数(HOMA-IR)差异,并以Pearson分析EAT与各指标的相关性。结果试验组EAT厚度、LDL-C、hs-CRP、HOMA-IR较对照组高,HDL-C较对照组低(P<0.05);Pearson分析结果显示,EAT厚度与LDL-C(r=-0.533)、hs-CRP(r=-0.529)、HOMA-IR(r=-0.594)呈正相关,与HDL-C(r=-0.489)呈负相关(P<0.05)。结论EAT厚度、LDL-C、hs-CRP、HOMA-IR、HDL-C等指标均参与高血压冠状动脉病变的发生、发展过程,且EAT厚度与高血压患者冠状动脉病变存在明显相关性,EAT越厚冠状动脉病变越严重。 Objective To analyze the correlation between epicardial adipose tissue(EAT)and coronary artery disease in patients with hypertension.Methods Totally 102 patients with hypertension and coronary artery disease in the First Affiliated Hospital of Xinxiang Medical College from January 2019 to December 2021 were selected,all of whom underwent coronary angiography and transthoracic Doppler ultrasound.According to the coronary angiography score(Gensini),45 patients were divided into the test group(Gensini score≥40)and 57 patients in the control group(Gensini score40).Compared the differences between the EAT thickness,low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG)Total cholesterol(TC),high-sensitivity C-reactive protein(hs CRP)and insulin resistance index(HOMA-IR).Pearson analysis was used to analyze the correlation between EAT and other indicators.Results EAT thickness,LDL-C,hs-CRP and HOMA-IR in experimental group were higher than those in control group,and HDL-C was lower than those in control group(P<0.05).Pearson analysis showed that EAT thickness was positively correlated with LDL-C(r=-0.533),hs-CRP(r=-0.529)and HOMA-IR(r=-0.594),and negatively correlated with HDL-C(r=-0.489)(P<0.05).Conclusion EAT thickness,LDL-C,hs-CRP,HOMA-IR,HDL-C and other indicators are involved in the occurrence and development of hypertension coronary artery disease,and there is a significant correlation between EAT thickness and hypertension patients with coronary artery disease,the thicker the EAT,the more serious the coronary artery disease.
