基于MR T2WI影像组学诊断早期糖尿病肾病 被引量:4

Radiomics based on MR T2WI for diagnosis of early diabetic nephropathy

摘要 目的观察基于MR T2WI影像组学诊断早期糖尿病肾病(DN)的价值。方法收集59例估算肾小球滤过率(eGFR)为60~120 ml/(min·1.73 m^(2))且尿白蛋白/肌酐比值(ACR)>30 mg/g的早期DN患者作为试验组,另以46名无肾脏疾病、心脏疾病、高血压及痛风且eGFR为80~120 ml/(min·1.73 m^(2))的健康志愿者为对照组。按照7∶3比例将全部受试者分为训练集(n=73)和测试集(n=32),训练集包括试验组40例、对照组33名,测试集包括试验组19例、对照组13名受试者。提取训练集右侧肾脏T2WI影像组学特征,并进行筛选,构建影像组学模型,观察其诊断早期DN的价值。结果试验组与对照组性别及年龄差异均无统计学意义(P均>0.05),而试验组尿素及肌酐均大于对照组(P均<0.05)。影像组学模型诊断训练集及测试集早期DN的敏感度、特异度及曲线下面积分别为95.20%、77.41%及0.892和88.23%、53.33%及0.765。结论基于MR T2WI影像组学诊断早期DN具有一定价值。 Objective To observe the value of radiomics based on MR T2WI for diagnosis of early diabetic nephropathy(DN).Methods Totally 59 early DN patients with estimated glomerular filtration rate(eGFR)60—120 ml/(min·1.73 m^(2))and urinary albumin-to-creatinine ratio(ACR)>30 mg/g were enrolled(experimental group),and 46 healthy volunteers with eGFR 80—120 ml/(min·1.73 m^(2))but without kidney disease,heart disease,hypertension and gout were taken as controls(control group).Then all the subjects were allocated to training set(n=73)and test set(n=32)at the ratio of 7∶3.Training set included 40 cases of experimental group and 33 healthy subjects,while test set included 19 cases of experimental group and 13 healthy subjects.The radiomic features of right kidney were extracted and screened based on T2WI to construct the radiomic model,and its value for diagnosis of early DN was observed.Results There was no significant difference of gender nor age between experimental group and control group(both P>0.05),while the urea and creatinine in experimental group were higher than those in control group(both P<0.05).The sensitivity,specificity and area under the curve of radiomic model for diagnosis of early DN in training set was 95.20%,77.41%and 0.892,in test set was 88.23%,53.33%and 0.765,respectively.Conclusion Radiomics based on MR T2WI had certain value for diagnosis of early DN.
