摘要
目的:探究早期整形修复对小儿手部烧伤后瘢痕挛缩畸形创面愈合及关节功能的影响。方法:选择2018年1月-2020年1月笔者医院收治的127例小儿手部烧伤患者临床资料,以随机数字法分为对照组(63例)及观察组(64例),对照组进行在烧伤10 d后进行修复手术及术后功能恢复锻炼,观察组在10 d内进行修复手术,并进行术后功能锻炼。比较两组患者术后1年临床疗效,于术前、术后1、6、12个月评定手指关节总活动度(TAM)、日常生活量表(ADL)评分,术后并发症发生率。结果:观察组临床疗效(96.88%)高于对照组(85.71%),差异有统计学意义(P<0.05)。两组患者在术后1、6、12个月ADL和TAM评分均明显高于术前,且观察组在术后6、12个月各评分均明显高于对照组(P<0.05)。观察组共出现2例并发症(3.13%),包括1例术后感染,1例植皮坏死;对照组患者出现7例(11.11%),包括3例术后感染,2例植皮坏死,2例植皮臃肿,组间数据相比差异有统计学意义(P<0.05)。结论:患儿对治疗配合度较低,易出现并发症,早期整形修复可有效降低手部烧伤患者术后瘢痕挛缩畸形、感染、植皮坏死等并发症发生率,明显改善手部关节功能的灵活性,有利于提高生活质量。
Objective To explore the effect of early plastic repair on wound healing and joint function of children with hand burn scar contracture deformity.Methods clinical data of 127 children with hand burn from January 2018 to January 2020 in our hospital were selected.They were randomly divided into control group (63 cases) and observation group (64 cases).The control group received repair operation and postoperative functional recovery exercise 10 days after burn,while the observation group received repair operation and postoperative functional exercise within 10 days.The total angle of motion (TAM),activity of daily living scale (ADL) score and postoperative complication rate were evaluated before operation,1 month,6 months and 12 months after operation.Results the clinical efficacy of the observation group (96.88%) was higher than that of the control group (85.71%) (P <0.05).The ADL and Tam scores of the two groups at 1,6 and 12 months after operation were significantly higher than those before operation,and the difference was statistically significant between the two groups at 6 months and 12 months after operation (P <0.05).There were 2 cases of complications (3.13%) in the observation group,including 1 case of postoperative infection and 1 case of skin graft necrosis.7 cases (11.11%) in the control group,including 3 cases of postoperative infection,2 cases of skin graft necrosis,2 cases of skin graft hypertrophy,the difference between the two groups was statistically significant (P <0.05).Conclusion Early plastic repair can effectively reduce the incidence of postoperative scar contracture deformity,infection,skin graft necrosis and other complications,significantly improve the flexibility of hand joint function and improve the quality of life.
