摘要
目的:分析序贯压力康复疗法联合负压创面治疗技术对手烧伤后瘢痕挛缩修复患者关节功能障碍及瘢痕情况的影响。方法:回顾性收集笔者科室2018年1月-2019年12月收治的120例手烧伤后瘢痕挛缩患者临床资料,根据治疗方法分为对照组和观察组,各60例。对照组采用负压创面治疗技术进行治疗,观察组采用负压创面治疗技术联合序贯压力康复疗法进行治疗,两组均治疗6个月。比较两组治疗6个月后的疗效,比较两组治疗前和治疗6个月后手指关节活动度、手功能、日常生活能力及瘢痕情况。结果:治疗6个月后,观察组总有效率为95.00%,显著高于对照组(81.67%),差异有统计学意义(P<0.05)。治疗6个月后,两组手指关节总活动度(Total activity of motion,TAM)评分、手功能评分、外观满意度评分、日常生活能力(Activities of daily living,ADL)各项评分较治疗前均升高,观察组高于对照组,差异有统计学意义(P<0.05)。治疗6个月后,两组瘢痕情况评分较治疗前降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:采用序贯压力康复疗法联合负压创面治疗技术对手烧伤后瘢痕挛缩患者进行治疗,可有效改善患者手关节功能障碍,提高患者日常生活能力及手外观满意度,同时改善患者瘢痕情况。
Objective To analyze the effect of sequential pressure rehabilitation therapy combined with negative pressure wound therapy on joint dysfunction and scar in patients with scar contracture after hand burn.Methods Clinical data of 120 patients with scar contracture after hand burn admitted to the Burn department of our hospital from January 2018 to December 2019 were collected retrospectively.According to the treatment methods,they were divided into the control group and the observation group,60 cases in each group.The control group was treated with negative pressure wound therapy,and the observation group was treated with sequential pressure rehabilitation therapy on the basis of the control group.Both groups were treated for 6 months.Compare the curative effect of the two groups after 6 months of treatment,compare the range of motion of finger joints,hand function,activities of daily living,scar before and after 6 months of treatment.Results After 6 months of treatment,the total effective rate in the observation group(95.00%)was significantly higher than the control group(81.67%),the difference was statistically significant(P<0.05).6 months after treatment,the scores of total range of motion(TAM),hand function,appearance satisfaction and activities of daily living(ADL)of the two groups were higher than those before treatment,and the observation group was higher than those of the control group(P<0.05).After 6 months of treatment,the scores of scar in both groups were lower than those before treatment,and the scores in the observation group were lower than the control group(P<0.05).Conclusion Sequential pressure rehabilitation therapy combined with negative pressure wound therapy to treat patients with scar contracture after hand burn can effectively improve the dysfunction of hand joint,improve the daily living ability of patients,improve the satisfaction of patients with hand appearance,and improve the scar situation of patients.
