摘要
[目的]介绍关节镜下"长袖套"保留残迹联合内减张前交叉韧带(anterior cruciate ligament,ACL)重建术的手术技术和初步临床效果.[方法]回顾性分析上述技术ACL重建治疗韧带上体部断裂且部分残端有附着点的95例患者的临床资料.将断裂的ACL残束制成"长袖套",将重建的移植物从中穿出,并联合使用关节内减张术重建ACL.术后行ADT、Lach-man试验评估韧带稳定性,单腿跳跃试验评估膝关节运动功能;并进行IKDC评分和Lysholm评分.术后2年行影像学检查和二次关节镜检,评估韧带生长情况.[结果]所有患者均得到2年以上临床随访,膝关节稳定性均得到了有效恢复,术后膝关节功能较术前有显著提高(P<0.05).二次镜检评估,韧带生长良好,无明显松弛.[结论]ACL"长袖套"保残重建联合关节内减张技术,手术操作简便有效,能显著促进韧带生长并改善膝关节功能.
[Objective]To introduce the surgical technique and preliminary clinical outcomes of arthroscopic anterior cruciate ligament(ACL)reconstruction with long-cuff remnant-preserving and graft tension-relieving.[Methods]A retrospective study was conducted on 95 patients who underwent abovementioned ACL reconstruction for upper part rupture of the ligament with residual upper adhesion.The teared ACL was trimmed and the remnant stump was made into a"long-cuff".Finally,the graft with enhancing suture was placed into the position passing through the"long cuff"and fastened with intraarticular tension relieving suture.Postoperatively,anterior drawer test,Lach-man test and single leg jump test,as well as IKDC Lysholm scores were used to evaluate the knee motor function.In addition,imaging examination and second-look arthroscopy were conducted 2 years after surgery to evaluate the ligaments healing.[Results]All patients were followed up for more than 2 years.The knee stability was effectively restored in all patients,associated with significant improvements in term of postoperative knee functional score compared to those preoperatively(P<0.05).The second-look arthroscopy revealed the reconstructed ligaments healed well without significant relaxation.[Conclusion]The arthroscopic anterior cruciate ligament reconstruction with long-cuff remnant-preserving and graft tension-relieving is a simple and effective technique,and does considerably promote ligament healing and improve knee function.
