摘要
目的比较行新鲜周期双卵裂期胚胎移植与单囊胚移植年轻患者的妊娠结局。方法回顾性分析采用卵泡期超长方案助孕并行新鲜周期双卵裂期胚胎移植与单囊胚移植的547例年轻患者临床资料。根据移植天数和移植数目分为移植双卵裂期胚胎组(A组)288例,单囊胚移植组(B组)259例,比较2组患者的基本情况、临床和实验室指标及妊娠结局。结果2组患者的hCG阳性率、临床妊娠率、生化妊娠率、早期流产率、晚期流产率、活产率、巨大儿以及新生儿畸形比较,差异均无统计学意义(P>0.05);A组的多胎妊娠率、早产率、低出生体重儿比例均高于B组,B组的胚胎种植率、总费用、婴儿出生体质量以及新生儿中男孩比例均高于A组,差异均有统计学意义(P<0.05)。结论在年轻患者(≤35岁)新鲜周期移植中,与双卵裂期胚胎移植相比,单囊胚移植在获得满意临床妊娠率和活产率的前提下,可以有效降低多胎妊娠率、早产率及低体重儿出生率,改善新生儿出生体重,获得稳定的妊娠结局及新生儿出生结局。
Objective To compare the pregnancy outcomes of young patients undergoing double cleavage embryo transfer and single blastocyst transfer in fresh cycle.Methods A retrospective analysis of 547 patients with double cleavage embryo transfer and single blastocyst transfer of fresh cycle undergoing long-term follicular phase therapy.Patients were divided into two groups according to different transfer days and transfer numbers,among which 288 cases were double cleavage embryo transfer(group A)and 259 cases were single blastocyst transfer(group B).The general characteristics,laboratory and pregnancy outcomes of the two groups were compared.Results There were no significant differences in hCG positive rate,clinical pregnancy rate,biochemical pregnancy rate,early abortion rate,late abortion rate,live birth rate,macrosomia and neonatal malformations in the two groups(P>0.05).The multiple pregnancy rate,premature birth rate,and low birth weight infants in group A were higher than those in group B(P<0.05).The embryo implantation rate,cost,birth weight and the proportion of boys in newborns in group B were higher than those in group A(P<0.05).Conclusions In the fresh cycle transfer of young patients(≤35 years old),compared with double cleavage embryo transfer,single blastocyst transfer can effectively reduce the multiple pregnancy rate,preterm birth rate and low birth weight infant birth rate,improve neonatal birth weight and obtain stable pregnancy outcome and neonatal birth outcome on the premise of satisfactory clinical pregnancy rate and live birth rate.
