摘要
目的:探究妊娠期高危型人乳头瘤病毒(HPV)感染对妊娠结局的影响。方法:选取2015年1月-2020年12月于本院确诊为高危型HPV感染的孕妇162例为感染组,同期产前检查未感染HPV的健康产妇162例作为对照组。比较两组产妇妊娠结局及产后情况,感染组HPV感染分型及其妊娠结局。结果:感染组高危型HPV感染分型包括16、18、31、33、35、39、45、51、52、56、58、59、66、68及82亚型,其中以52亚型(19.8%)及16亚型为主(14.8%),感染组HPV 52亚型、HPV 16亚型、其余亚型及≥2种混合感染孕妇的不良妊娠结局比较无差异(P>0.05),早产(8.6%)及胎膜早破(12.4%)发生率高于对照组(2.5%、2.5%)(P<0.05);两组产妇产后出血及产褥感染发生无差异(P>0.05)。结论:孕期高危型HPV感染常见亚型为HPV 52亚型及HPV 16亚型,HPV感染会增加早产及胎膜早破发生,有必要做好孕前及妊娠期HPV筛查及干预。
Objective:To study the influence of high risk HPV infection of women during pregnancy on their pregnancy outcomes.Methods:162 pregnant women diagnosed by HPV infection were selected in research group,and another 162 healthy pregnant women were selected in control group from January 2015 to December 2020.The pregnancy outcomes and postpartum situation of the women were compared between the two groups,and the HPV infection types and the pregnancy outcomes of the women in the research group were analyzed.Results:The high-risk HPV infection types of the women in the research group included subtypes 16,18,31,33,35,39,45,51,52,56,58,59,66,68,and 82,and the majority of them were subtype 52(19.8%)and subtype 16(14.8%).There was no significant difference in adverse pregnancy outcomes rate among the women with HPV 52 subtype infection,women with HPV 16 subtype infection,women with other subtypes infection,and women with≥2 subtypes of mixed infection in the research group(P>0.05).The rates of preterm birth(8.6%)and premature rupture of membranes(12.4%)of the women in the research group were significant higher than those(2.5%and 2.5%)of the women in the control group(P<0.05).The incidences of postpartum hemorrhage and puerperal infection of the women had no significant difference between the two groups(P>0.05).Conclusion:The common HPV subtypes of the pregnant women with high risk HPV infection during pregnancy were HPV 52 and HPV 16.HPV infection can increase the incidence of adverse pregnancy outcomes and premature rupture of membranes.Therefore,it is necessary to screen and intervene HPV infection of women pre pregnancy and during pregnancy.
