摘要
目的探讨外周血Toll样受体9(TLR9)基因多态性与成人EB病毒(EBV)感染风险及免疫功能的关系。方法选择南京医科大学附属常州市第二人民医院2018年1月-2020年1月收治的EB病毒感染患者120例为研究组,选择同期医院进行体检的健康人70名为对照组。针对TLR9基因1486T/C设计特异性引物,采用高分辨率熔解曲线法(HRM)检测基因分型。采用流式细胞仪对EB病毒感染患者外周血中CD_(4)^(+)、CD_(4)^(+)/D_(8)^(+)、NK细胞、B细胞占比进行检测。结果120例EBV感染阳性患者以传染性单核细胞增多症为主,共85例,占70.83%,其次是噬血细胞综合征、恶性肿瘤和恶性血液病,分别占10.00%、8.33%、6.67%。研究组TLR9基因1486T/C位点TT基因型、T等位基因频率(24.17%、47.50%)低于对照组(P<0.05)。TLR9-1486T/C位点TC/CC型患者CD_(4)^(+)、CD_(4)^(+)/D_(8)^(+)、NK细胞、B细胞水平(27.10±2.58%、1.65±0.19、0.31±0.07%、0.41±0.08%)低于TT型患者;TLR9-1486T/C位点不同基因型疾病类型比较差异无统计学意义;多元Logistic回归分析结果显示,TLR9TC/CC基因型、TLR9C等位基因是成人EB病毒感染的独立危险因素(P<0.05)。结论TLR9基因1486T/C位点TC、CC基因型增加本地区人群对EBV的易感性,但与疾病种类无关。
OBJECTIVE To explore the relationship between the peripheral blood Toll-like receptor 9(TLR9)gene polymorphism and risk of Epstein-Barr virus(EBV)infection as well as immune function.METHODS A total of 120patients with EBV infection who were treated in Changzhou No.2People′s Hospital Affiliated to Nanjing Medical University from Jan 2018to Jan 2020were assigned as the study group,meanwhile,70healthy people who received physical examination were set as the control group.Specific primers were designed for TLR9gene 1486T/C,and the high resolution melting curve(HRM)method was used to detect the genotyping of this locus,and the percentages of CD_(4)^(+),CD_(4)^(+)/D_(8)^(+),NK cells and B cells in peripheral blood were detected by means of flow cytometry.RESULTS Among the 120patients with EBV infection,85had infectious mononucleosis,accounting for 70.83%,the patients with hemophagocytic syndrome 10.00%,the patients with malignant tumors 8.33%,the patients with hematological malignancies 6.67%.The frequencies of TT genotype and T allele at1486T/C locus of TLR9gene were respectively 24.17%and 47.50%in the study group,significatly lower than those in the control group(P<0.05).The levels of CD_(4)^(+),CD_(4)^(+)/D_(8)^(+),NK cell and B cell of the patients with TC/CC genotype at TLR9-1486T/C locus were respectively 27.10±2.58%,1.65±0.19,0.31±0.07%and 0.41±0.08%,significatly lower than those of the patients with TT genotype;there was no significant difference in the type of disease among the patients with different genotypes at TLR9-1486T/C locus.The result of multivariate logistic regression analysis showed that TLR9TC/CC genotype and TLR9Callele were the independent risk factors for the EBV infection in the adults(P<0.05).CONCLUSIONThe TC and CC genotypes at 1486T/C locus of TLR9gene may increase the susceptibility to EBV in this area but are not associated with the type of disease.
