负荷超声心动图评价急进海拔5000m以上人员右心功能及右心收缩储备能力变化的研究

摘要 目的:应用负荷超声心动图评价急进海拔5000 m以上人员右心功能及右心收缩储备能力变化情况。方法:选取由平原地区急进高原地区的健康男性青年官兵60例。所有受试者均在平原地区及进入高原地区1周后进行负荷超声心动图(stress echocardiography,SE)检查,测量静息状态下及负荷峰值状态下右心功能参数,包括:三尖瓣环收缩期位移(tricuspid annular plane systolic excursion,TAPSE)、右心室TEI指数(right ventricle-TEI,RV-TEI)、右室面积变化分数(fraction of right ventricular area changes,FAC)以及组织多普勒三尖瓣环收缩期峰值速度(tissue Doppler tricuspid annular plane systolic velocity,V);同时根据测量到的静息状态及负荷峰值状态下右心功能参数,计算出平原及高原地区右心收缩储备能力参数。根据海拔高度不同,将检测数据分为平原组与高原组。结果:急进高原后静息状态及峰值负荷状态下右心功能参数TAPSE、FAC、V均显著低于急进高原前,RV-TEI显著高于平原组,差异有显著统计学意义(P<0.01);急进高原后右心收缩储备能力TAPSE、FAC、V显著低于急进高原前,差异有显著统计学意义(P<0.01)。结论:急进海拔5000 m以上高原地区人员右心适应能力及储备能力较平原地区有显著降低,右心系统及肺血管重塑风险增加。 Objective:Evaluation the changes of right ventricular function and contractile reserve capacity in subjects who exposed above 5000 m by stress echocardiography.Methods:Sixty male subjects were chosen in this research.All the subjects were performed stress echocardiography(SE)in plain and 1 week after exposed in plateau.Right ventricular function parameters were measured under rest and peak-loading state by SE,which including tricuspid annular plane systolic excursion(TAPSE),right ventricle-TEI(RV-TEI),fraction of right ventricular area changes(FAV)and tissue Doppler tricuspid annular plane systolic velocity(V)then the parameters of right ventricular contractile reserve capacity in plain and plateau could be calculated.All data were separated into plain and plateau group according to the altitude.Results:The TAPSE,FAC and V after entry into plateau were significantly lower than before entry into plateau under rest and peak-loading state(P<0.01).The right ventricular contractile reserve capacity parameters after entry into plateau were significantly lower than before entry into plateau(P<0.01).The RV-TEI after entry into plateau was significantly higher than before entry into plateau under rest and peak-loading state(P<0.01).Conclusion:The right ventricular function and contractile reserve capacity were significant lower than acute exposed above 5000 m,which accompany with higher risk of right cardiac and pulmonary vascular system remodeling.
