单兵高压氧舱对急进高海拔地区人员右心功能及右心收缩储备能力的影响 被引量:3

Effect of individual hyperbaric chamber on right ventricular function and right heart systolic reserve capacity in serviceman advancing rapidly to high altitudes

摘要 目的:研究单兵高压氧舱对急进海拔5000 m以上人员右心功能及右心收缩储备能力的影响。方法:选取由乌鲁木齐地区(海拔890 m)乘车急进喀喇昆仑山区(海拔5137 m)的健康青年男性官兵90例,随机分为对照组、低氧组、高压氧组3组,每组30例。对照组急进高原后不给予任何干预;低氧组每日给予低流量氧疗;高压氧组每日给予单兵高压氧舱治疗。急进高原1个月后,对3组受试者进行负荷超声心动图(stress echocardiography,SE)检查,分别测量每例受试者在静息状态和峰值负荷状态下的三尖瓣环收缩期位移(tricuspid annular plane systolic excursion,TAPSE)、右室面积变化分数(fraction of right ventricular area changes,FAC)、组织多普勒三尖瓣环收缩期峰值速度V以及右心室Tei(right ventricle-Tei,RV-Tei)指数等右心功能参数,再计算出相应的右心收缩储备能力参数。测量数据由SPSS 19.0软件进行统计学分析。结果:高压氧组在静息状态与峰值负荷状态下的TAPSE、FAC、V均显著高于对照组、RV-Tei指数显著低于对照组,差异有统计学意义(P<0.01);高压氧组在静息状态与峰值负荷状态下的TAPSE、FAC显著高于低氧组,差异有统计学意义(P<0.01)。高压氧组右心收缩储备能力参数均显著高于对照组,差异有统计学意义(P<0.01);高压氧组右心储备能力参数FACs显著高于低氧组,差异有统计学意义(P<0.01)。结论:单兵高压氧舱可有效提高急进高原人员的右心功能及右心收缩储备能力,对提高急进高原人员工作效能、预防高原性心脏病的发生具有十分重要的意义。 Objective To evaluate the effect of the individual hyperbaric chamber on right ventricular function and right heart systolic reserve capacity in the serviceman advancing rapidly to altitudes above 5000 m.Methods Ninety healthy young male servicemen were selected from the Urumqi area(890 m above sea level)who had been rushed into the Karakorum Mountains(5137 m above sea level)by vehicle,and were randomly divided into three groups:a control group,a low oxygen group and a hyperbaric group,with 30 cases in each group.In the control group,no intervention was given after the acute entrance to the plateau;in the low oxygen group,low-flow oxygen therapy was performed daily;in the hyperbaric group,individual hyperbaric chamber treatment was carried out daily.One month after rushing into the plateau,three groups of subjects underwent stress echocardiography(SE)examinations to measure tricuspid annular plane systolic excursion(TAPSE),fraction of right ventricular area changes(FAC),tissue Doppler peak systolic velocity of tricuspid annular(V)and right ventricular function indexes such as right ventricle-Tei(RV-Tei)at rest and at peak load states,and the corresponding parameters for right heart systolic reserve capacity were calculated.SPSS 19.0 software was used for statistical analysis.Results TAPSE,FAC and V were significantly higher in the hyperbaric group than in the control group at both rest and peak load states,and RV-Tei was significantly lower than in the control group,with statistically significant differences(P<0.01);TAPSE and FAC were significantly higher in the low oxygen group than in the low oxygen group at both rest and peak load states,with statistically significant differences(P<0.01).The right heart systolic reserve capacity parameters were all significantly higher in the hyperbaric group than in the control group,with statistically significant differences(P<0.01);the right heart reserve capacity parameters FACswere significantly higher in the hyperbaric group than in the low oxygen group,with statisticall
