摘要
目的探讨淋巴细胞与单核细胞比值(LMR)、髓样细胞白血病蛋白1(MCL1)、Dickkopf-1(DKK1)与多发性骨髓瘤(MM)患者骨损伤、预后相关性。方法选取安徽省皖北煤电集团总医院血液内科118例MM患者作为研究对象,比较不同临床病理特征患者LMR、MCL1、DKK1表达,分析上述指标与多发性骨髓瘤骨病(MBD)分级相关性,评价上述指标不同表达与预后的相关性,探讨MM患者预后不良的影响因素。结果 MCL1、DKK1与MBD分级呈正相关,LMR与MBD分级呈负相关(P<0.05);LMR低表达、MCL1高表达、DKK1高表达患者3年生存率低于LMR高表达、MCL1低表达、DKK1低表达患者,差异有统计学意义(P<0.05);MBD分级、Hb、LMR、MCL1、DKK1是MM患者预后的影响因素(P<0.05)。结论 LMR、MCL1、DKK1与MM患者骨损伤存在一定相关性,且均为患者预后的影响因素,可为临床制定个性化治疗方案、预测预后提供新思路。
Objective To investigate the correlation of lymphocyte to monocyte ratio(LMR),myeloid leukemia protein 1(MCL1),Dickkopf-1(DKK1) with bone injury and prognosis in patients with multiple myeloma(MM).Methods A total of 118 patients with MM in the Department of Hematology,Wanbei Coal and Electricity Group General Hospital in Anhui Province were selected as the research objects.The expressions of LMR,MCLl and DKKl in patients with different clinicopathological characteristics were compared,and the correlation between the above indicators and multiple myeloma bone disease(MBD) grades was analyzed,and the correlation between the different expressions of the above indicators and prognosis was evaluated,The factors influencing the prognosis of MM patients were evaluated.Results MCLl and DKK1 were positively correlated with MBD grade,and LMR was negatively correlated with MBD grade(P<0.05).The 3-year survival rate of patients with low LMR,high MCL1 DKK1 expression was lower than that of patients with high LMR, low MCL1, and DKK1 Low expression(P<0.05).MBD grade,Hb,LMR,MCL1,DKK1 are the prognostic factors of MM patients(P<0.05).Conclusion There is a certain correlation between LMR,MCL1, DKK1 and bone injury in MM patients,and they are all factors that affect the prognosis of patients.They can provide new ideas for clinical development of personalized treatment and prediction of prognosis.
