摘要
目的探讨多发性骨髓瘤(multiple myeloma,MM)患者血清miRNA-181a(miR-181a)水平与病理特征的关系。方法选取2013年4月~2015年4月我院收治的MM患者50例作为MM组,收集患者的病理特征资料,包括性别、年龄、体质指数、首诊症状、骨髓瘤骨病(myeloma bone disease,MBD)分级、国际分期系统(international staging system,ISS)分期、体力状况评分[采用美国东部肿瘤协作组(Eastern Cooperative Oncology Group,ECOG)评分标准]、分型以及血清miRNA-181a水平。选取同期于我院体检的体检者50例作为体检组,比较两组血清miRNA-181a水平,采用受试者工作特征曲线(receiver operating characteristic,ROC)分析血清miRNA-181a对MM的预测价值。利用Spearman检验分析血清miRNA-181a与病理特征的关系。结果 MM组血清miR-181a水平高于体检组,差异有统计学意义(P<0.05)。血清miR-181a预测MM的曲线下面积为0.668(标准误=0.053,P=0.004,95%CI=0.563~0.772)。血清miRNA-181a低表达者的骨病A级、ISSⅠ期占比分别为58.33%、58.33%,高于miRNA-181a高表达者的10.53%、7.89%,差异有统计学意义(P<0.05)。Spearman检验提示血清miRNA-181a表达水平与MM患者的骨病MBD分级、ISS分期呈正相关(P<0.05)。结论与健康体检者相比,MM患者的血清miRNA-181a水平明显增高,且与骨病MBD分级、ISS分期有相关性。
Objective To investigate the relationship between serum miRNA-181a(miR-181a) level and pathological features in patients with multiple myeloma(MM). Methods 50 patients with MM admitted to our hospital from April 2013 to April 2015 were selected as the MM group. The pathological characteristics of the patients were collected, including gender, age, body mass index, first diagnosis, myeloma bone disease(MBD) grading, international staging system(ISS) staging, physical status score[using Eastern Cooperative Oncology Group(ECOG)scoring criteria], typing, and serum miRNA-181a levels. 50 patients with physical examination in our hospital were selected as the physical examination group. Serum miRNA-181a levels were compared between the two groups. The receiver operating characteristic(ROC) was used to analyze the predictive value of serum miRNA-181a for MM. The relationship between serum miRNA-181a and pathological features was analyzed by Spearman test. Results The serum miR-181a level in the MM group was higher than that in the physical examination group, and the difference was statistically significant(P<0.05). The area under the curve of serum miR-181a predicted MM was 0.668(standard error=0.053,P=0.004,95%CI=0.563-0.772). The percentages of bone disease grade A and ISS stage Ⅰ in low expression of miRNA-181a patients were 58.33% and 58.33%, respectively,which were higher than those of miRNA-181a high expression patients(10.53%, and 7.89%). The difference was statistically significant(P<0.05). Spearman test showed that serum miRNA-181a expression level was positively correlated with MBD grade and ISS stage of MM patients(P<0.05). Conclusion Compared with healthy subjects, serum miRNA-181a level is significantly higher in MM patients and it is correlated with MBD grading and ISS staging.
