摘要
目的观察参附注射液联合机械通气治疗重症哮喘合并Ⅱ型呼吸衰竭的临床疗效。方法将我院重症哮喘合并Ⅱ型呼吸衰竭患者106例随机分为对照组与观察组各53例。对照组给予机械通气治疗,观察组在对照组治疗基础上联合参附注射液治疗。比较两组临床疗效,治疗前、治疗后1周生命指标、血气分析指标、血清炎性指标、肺功能指标以及体液免疫指标。结果观察组临床疗效显著高于对照组(P<0.05);治疗后,两组患者心率(HR)、呼吸频率(RR)、二氧化碳分压(PaCO_(2))、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白介素-8(IL-8)、白介素-17(IL-17)水平较治疗前均降低,且观察组均低于对照组(P<0.05);两组患者氧分压(PaO_(2))、峰值流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、1秒率(FVC/FEV1)及免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)水平较治疗前升高且观察组均高于对照组(P<0.05)。结论参附注射液联合机械通气治疗重症哮喘合并Ⅱ型呼吸衰竭患者效果良好,可有效改善其肺功能及生命体征,降低炎症因子水平,提高机体免疫力。
Objective To observe the clinical efficacy of Shenfu Injection combined with mechanical ventilation in treatment of severe asthma with type Ⅱ respiratory failure.Methods A total of 106 patients with severe asthma combined with type Ⅱ respiratory failure were randomly divided into control group and observation group,with 53 cases each.Control group was treated with mechanical ventilation and observation group was treated with Shenfu Injection on the basis of control group.Both groups were treated continuously for 1 week,the clinical efficacy,vital indicators,blood gas analysis indicators,serum inflammatory indicators,lung function indicators and humoral immunity indicators were compared between the two groups.Results The clinical efficacy of observation group was higher than that of control group,the difference was statistically significant(P<0.05);after treatment,heart rate(HR),respiratory rate(RR) partial pressure of carbon dioxide(PaCO_(2)),calcitonin(PCT),hypersensitive C-reactive protein(HS-CRP),interleukin-8(IL-8),and interleukin-17(IL-17) levels in both groups were all lower than those before treatment,and the levels in observation group were lower than those in control group(P<0.05);the levels of oxygen partial pressure(PaO_(2));peak flow rate(PEF),force vital capacity(FVC),forced expiratory volume in the first second(FEV1),one-second rate(FVC/FEV1),and immunoglobulin G(IgG),IgM and IgA in both groups were increased compared with those before treatment,and the levels of IgG,IgM and IgA in observation group were higher than those in control group(P<0.05).Conclusion Shenfu Injection combined with mechanical ventilation has a good effect in treatment of severe asthma patients with type Ⅱ respiratory failure,which can effectively improve the lung function and vital signs,reduce the level of inflammatory factors,and improve the immunity.
