无创正压通气对重症支气管哮喘合并Ⅱ型呼吸衰竭的应用效果及并发症发生率影响评价

Evaluation of the application effect and incidence of complications of non-invasive positive pressure ventilation on severe bronchial asthma complicated with type Ⅱ respiratory failure

摘要 目的研究无创正压通气对重症支气管哮喘合并Ⅱ型呼吸衰竭的应用效果以及并发症发生率影响效果.方法选择广水市第二人民医院在2021年1月-2023年12月期间收治的80例重症支气管哮喘合并Ⅱ型呼吸衰竭患者,按照随机数字表法分组,对照组(n=40)实施常规治疗,包括补液、吸氧、给予糖皮质激素和支气管扩张剂,观察组(n=40)以对照组疗法为基础,实施无创正压通气技术治疗,比较两组患者治疗前后血气分析指标、治疗前后凝血功能指标、治疗前后免疫炎性反应和氧化应激指标、治疗前后呼吸力学指标及不良反应发生率.结果治疗前,两组患者血气分析指标、凝血功能指标、免疫炎性反应和氧化应激指标、呼吸力学指标比较,无明显差异(P>0.05),治疗后,观察组上述指标均优于对照组(P<0.05);观察组不良反应发生率和对照组比较,无明显差异(P>0.05).结论重症支气管哮喘合并Ⅱ型呼吸衰竭患者通过无创正压通气技术治疗,可改善其凝血、炎症反应、呼吸力学、血气指标,安全性良好,值得推广. Objective To study the application effect of non-invasive positive pressure ventilation on severe bronchial asthma complicated with typeⅡrespiratory failure and the impact on the incidence of complications.Methods 80 patients with severe bronchial asthma complicated with typeⅡrespiratory failure admitted to our hospital from January 2021 to December 2023 were randomly divided into two groups using a random number table method.The control group(n=40)received routine treatment,including fluid replacement,oxygen inhalation,glucocorticoids,and bronchodilators.The observation group(n=40)received non-invasive positive pressure ventilation technology treatment based on the control group therapy,and the blood gas analysis indicators of the two groups of patients before and after treatment were compared;coagulation function indicators before and after treatment;Immune inflammatory response and oxidative stress indicators before and after treatment;respiratory mechanics indicators before and after treatment;the incidence of adverse reactions.Results Before treatment,blood gas analysis indicators were obtained for both groups of patients;coagulation function indicators;immune inflammatory response and oxidative stress indicators;the comparison of respiratory mechanics indicators(P>0.05)showed that after treatment,the observation group had better indicators than the control group(P<0.05);the incidence of adverse reactions in the observation group was compared with that in the control group(P>0.05).Conclusion Non invasive positive pressure ventilation can improve coagulation,inflammatory response,respiratory mechanics,and blood gas indicators in patients with severe bronchial asthma combined with typeⅡrespiratory failure.The safety is good and worthy of promotion.
