摘要
目的:探讨柴胡舒肝散联合四联疗法对胃溃疡伴幽门螺杆菌(Helicobacter pylori,Hp)感染患者疗效、生活质量及血清学指标的影响。方法:选取2019年6月-2020年3月期间平顶山煤业(集团)十二矿职工医院收治的胃溃疡伴Hp感染患者90例作为研究对象,采用简单随机化法分为四联组(45例)和联合组(45例)。四联组接受四联疗法(阿莫西林+克拉霉素+奥美拉唑+胶体果胶铋)治疗,联合组接受柴胡舒肝散联合四联疗法治疗,对比两组Hp根除率、治疗有效率、血清学指标[C-反应蛋白(CRP)、白细胞介素-6(IL-6)、α-肿瘤坏死因子(TNF-α)]、生活质量及不良反应发生率。结果:联合组Hp根除率高于四联组(93.33%vs 75.56%),治疗有效率高于四联组(95.56%vs 77.78%),差异有统计学意义(P<0.05);治疗后两组CRP、IL-6和TNF-α水平较治疗前降低,且联合组低于四联组,差异有统计学意义(P<0.05);联合组生活质量各项评分均高于四联组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:对伴HP感染的胃溃疡患者使用柴胡舒肝散联合四联疗法进行治疗,可以提高治疗效果,降低炎症因子水平,提高患者生活质量,安全性好,具有临床应用价值。
Objective: To investigate the effect of Chaihu Shugan powder combined with quadruple therapy on the efficacy, quality of life and serological indexes of gastric ulcer patients with Helicobacter pylori(Hp) infection. Methods: From June 2019 to March 2020, 90 patients with gastric ulcer and Hp infection were selected. They were randomly divided into the quadruple group(n = 45) and the combined group(n = 45). The quadruple group was treated with quadruple therapy(amoxicill in+clarithromycin+omeprazole+colloidal bismuth pectin), and the combined group was treated with Chaihu Shugan powder combined with quadruple therapy. The eradication rate, effective rate, serum markers [C-reactive protein(CRP), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α) ], quality of life and the incidence of adverse reactions were compared between the two groups. Results: The Hp eradication rate of the combined group was higher than that of the quadruple group(93.33% vs 75.56%), and the effective rate of the combined group was higher than that of the quadruple group(95.56% vs 77.78%), the difference was statistically significant(P<0.05);After treatment, the levels of CRP, IL-6 and TNF-α in the two groups were lower than those before treatment, and the combined group was lower than that in the quadruple group, the difference was statistically significant(P < 0.05);The scores of quality of life in the combined group were higher than those in the quadruple group, the difference was statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P > 0.05). Conclusion: Chaihu Shugan powder combined with quadruple therapy in the treatment of gastric ulcer patients with HP infection can improve the treatment effect, reduce the level of inflammatory factors, improve the quality of life of patients, with good safety and clinical application value.
