摘要
目的探讨调肝活血汤辅助西药治疗肝火亢盛证顽固性高血压患者的疗效。方法选取2019年1月至2020年9月平顶山煤业(集团)十二矿职工医院收治的88例肝火亢盛证顽固性高血压患者作为研究对象,按照治疗方法分为西药组与中西药组,各44例。西药组接受常规西药治疗,中西药组在西药组基础上加用调肝活血汤治疗。比较两组疗效、血压[舒张压(DBP)、收缩压(SBP)]、中医症状积分、血管内皮功能[一氧化氮(NO)、人内皮素-1(ET-1)]。结果中西药组总有效率(93.18%)高于西药组(75.00%),差异有统计学意义(P<0.05)。治疗前,两组DBP、SBP水平比较,差异无统计学意义(P>0.05);治疗后,两组DBP、SBP水平均降低,且中西药组低于西药组,差异有统计学意义(P<0.05)。治疗前,两组中医症状积分比较,差异无统计学意义(P>0.05);治疗后,两组中医症状积分均降低,且中西药组低于西药组,差异有统计学意义(P<0.05)。治疗后,两组ET-1、NO水平均改善,且中西药组血清ET-1水平低于西药组,NO水平高于西药组,差异有统计学意义(P<0.05)。结论调肝活血汤辅助治疗肝火亢盛证顽固性高血压患者效果显著,可有效改善血压水平,缓解临床症状,提高血管内皮功能。
Objective To explore the curative effect of Tiaogan Huoxue Decoction in adjuvant treatment of refractory hypertension with hyperactive liver fire syndrome.Methods From January 2019 to September 2020,88 patients with refractory hypertension with hyperactive liver fire syndrome were selected as the research objects,and were divided into western medicine group and Chinese and Western medicine group according to treatment methods,with 44 cases in each group.Western medicine group received with conventional western medicine.Chinese and Western medicine group received Tiaogan Huoxue Decoction on the basis of the western medicine group.The efficacy,blood pressure[diastolic blood pressure(DBP),systolic blood pressure(SBP)],TCM symptom score,vascular endothelial function[nitric oxide(NO),human endothelin-1(ET-1)]were compared between the two groups.Results The total effective rate of Chinese and western medicine group(93.18%)was higher than that of the western medicine group(75.00%)(P<0.05).Before treatment,there was no difference in the levels of DBP and SBP between two groups(P>0.05).After treatment,the levels of DBP and SBP in two groups were decreased,and the levels of Chinese and western medicine group were lower than that of western medicine group(P<0.05).Before treatment,there was no difference in TCM symptom score between the two groups(P>0.05).After treatment,the TCM symptom score of both groups was decreased,and Chinese and western medicine group was lower than the western medicine group,(P<0.05).After treatment,the levels of ET-1 and NO were improved in both groups,and the serum ET-1 level in Chinese and western medicine group was lower than that in the western medicine group,and the NO level was higher than that in the western medicine group(P<0.05).Conclusion Tiaogan Huoxue Decoction is effective in adjuvant treatment of refractory hypertension patients with hyperactive liver fire,which can effectively improve blood pressure,relieve clinical symptoms,and improve vascular endothelial function.
