摘要
目的研究参芪抑瘤方联合幽门螺杆菌(Hp)根治方案对Hp阳性胃癌前病变(PLGC)患者的治疗效果,以及对血清胃泌素(GAS)、硫氧还蛋白-1(TRX-1)水平和胃黏膜C-myc蛋白表达的影响。方法选择2016年8月-2018年7月期间收治的180例Hp阳性PLGC患者,按照随机数字表法将患者分为对照组和观察组,每组90例。对照组患者接受Hp根治方案治疗:克拉霉素与阿莫西林连续治疗2周,奥美拉唑与枸橼酸铋钾连续治疗12周。观察组在对照组的基础上联合参芪抑瘤方,连续治疗12周。比较两组治疗前后血清中GAS、TRX-1水平、胃黏膜中C-myc蛋白表达、病理组织学评分、中医症候评分、临床功能(PRO)评分和疗效。结果观察组总有效率和Hp消除率为90.00%(81/90)、94.44%(85/90)高于对照组总有效率的76.67%(69/90)、84.44%(76/90),差异有统计学意义(P<0.05);治疗后,观察组血清GAS、TRX-1水平、胃黏膜C-myc蛋白表达均低于对照组,差异有统计学意义(P<0.05);观察组各项中医症候评分、各项PRO量表评分以及病理组织学评分低于对照组,差异有统计学意义(P<0.05)。结论采用参芪抑瘤方联合Hp根治方案治疗Hp阳性PLGC患者,能够显著提高疗效和Hp清除率,并可抑制血清中GAS、TRX-1和胃黏膜中C-myc蛋白的表达,改善患者临床表现和胃黏膜病变情况,值得推广应用。
Objective To study the therapeutic effect of Shenqi Yiliu Decoction combined with HP radical therapy on patients with HP-positive precancerous lesions of gastric cancer(PLGC),as well as the effects of serum gastrin(GAS),thioredoxin-1(TRX-1)levels and C-myc protein expression in gastric mucosa.Methods 180 cases of Hp-positive PLGC patients admitted to our hospital from August 2016 to July 2018 were divided into control group and observation group according to random number table method,90 cases in each group.The control group received HP radical therapy:Clarithromycin and Amoxicillin for 2 weeks,Omeprazole and Bismuth Potassium Citrate for 12 weeks.The observation group was treated with Shenqi Yiliu Decoction for 12 weeks on the basis of the control group.The levels of GAS and TRX-1 in serum,C-myc protein expression in gastric mucosa,histopathological score,TCM syndrome score,clinical function(PRO)score and curative effect were compared between the two groups before and after treatment.Results The total effective rate and the elimination rate of Hp in the observation group were 90.00%(81/90)and 94.44%(85/90)higher than 76.67%(69/90)and 84.44%(76/90)in the control group(P<0.05);After treatment,the levels of serum GAS,TRX-1 and C-myc protein expression in gastric mucosa in the observation group were lower(P<0.05);The scores of TCM symptoms,PRO scale and histopathology in the observation group were lower(P<0.05).Conclusion Shenqi Yiliu Decoction combined with HP radical therapy for Hp-positive PLGC patients can significantly improve the efficacy and clearance rate of Hp,inhibit the expression of GAS,TRX-1 in serum and C-myc protein in gastric mucosa,improve the clinical manifestations of patients and gastric mucosal lesions,which is worthy of popularization and application.
