摘要
目的:探讨冠心病患者血浆IL-18、MMP-22和MMP-29的表达水平与老年冠心病(CHD)冠状动脉病变的相关性。方法:选择120例行冠脉造影检查的老年患者,根据CAG检查和临床诊断结果将患者分为冠心病(CHD)组(76例)和非冠心病(NCHD)组(44例)。分析血浆IL-18、MMP-22和MMP-29在CHD不同冠脉病程度患者之间的差异及与SYNTAX积分、冠脉病变支数、冠脉狭窄率和CHD发病之间的关系,ROC曲线分析血浆IL-18、MMP-22和MMP-29对CHD的诊断价值。结果:CHD组血浆IL-18、MMP-22和MMP-29水平明显高于NCHD组(P<0.05)。CHD患者血浆中IL-18、MMP-22和MMP-29水平与冠脉病变支数、冠脉狭窄率、SYNTAX积分均呈正相关(P<0.05)。回归分析结果表明IL-18、MMP-22和MMP-29是CHD发病的危险因素。MMP-22和MMP-29预测老年CHD发病的ROC曲线下面积(AUC)最高达0.917,100.00%、93.24%(MMP-22)和84.61%、92.16%(MMP-29)。结论:老年CHD患者血浆IL-18、MMP-22和MMP-29水平明显升高,且其水平与冠脉病变支数、狭窄程度、SYNTAX积分均存在不同程度的相关性,IL-18、MMP-22及MMP-29可作为评价冠脉病变程度的指标。
Objective:To investigate the correlation between expression levels of IL-18,MMP-22 and MMP-29 and coronary artery disease in elderly patients with coronary heart disease(CHD).Methods:120 elderly patients with coronary angiography were selected and divided into coronary heart disease(CHD)group(76 cases)and non coronary heart disease(NCHD)group(44 cases)according to CAG examination and clinical diagnosis results. Objective to analyze the differences of plasma IL-18,MMP-22 and MMP-29 in patients with CHD of different degrees of coronary artery disease and the relationship with SYNTAX score,number of coronary artery lesions,coronary stenosis rate and incidence of CHD. ROC curve was used to analyze diagnostic value of plasma IL-18,MMP-22 and MMP-29 in diagnosis of CHD.Results:Levels of IL-18,MMP-22 and MMP-29 in CHD group were significantly higher than those in NCHD group(P<0.05). Levels of IL-18,MMP-22 and MMP-29 were positively correlated with the number of coronary artery lesions,coronary stenosis rate and syntax score(P<0.05). Regression analysis showed that IL-18,MMP-22 and MMP-29 were risk factors for CHD. ROC analysis showed that the area under the ROC curve(AUC)of MMP-22 and MMP-29 were 0.917,and the sensitivity and specificity were 100.00%,93.24%(MMP-22)and 84.61%,92.16%(MMP-29),respectively.Conclusion:Plasma levels of IL-18,MMP-22 and MMP-29 in elderly patients with CHD were significantly increased,and levels of IL-18,MMP-22 and MMP-29 were positively correlated with the number of coronary artery lesions,degree of stenosis and SYNTAX score,which can be used as biological indicators for early diagnosis of elderly CHD.
