摘要
目的:分析注射用血塞通冻干粉针剂联合尼可地尔对冠心病心绞痛患者心功能及CD4^+/CD8^+水平的影响。方法:选取2016年1月~2018年3月我院收治的98例冠心病心绞痛患者,随机数字表法分为对照组(n=49)、观察组(n=49),对照组予以常规综合治疗,观察组在此基础上予以注射用血塞通冻干粉针剂联合尼可地尔治疗,两组均治疗2个疗程,对比两组临床症状疗效、心电图疗效、心排血指数(CI)、每搏量(SV)、左室射血分数(LVEF)、心房收缩期二尖瓣血流速度比值(E/A)、心排血量(CO)、T细胞亚群(CD3^+、CD4^+/CD8^+)、纤维蛋白原(FIB)、血管性血友病因子(vWF)、D二聚体(D-D)、纤溶酶原抑制物-1(PAI-1)、组织型纤溶酶原激活剂(tPA)、肱动脉血流介导的内皮依赖性血管舒张功能(FMD)水平。结果:观察组临床症状疗效与心电图疗效总有效率分别为95.92%、79.59%,高于对照组77.55%、57.14%(P<0.05);观察组治疗后CI、LVEF、SV、CO、E/A、CD3^+、CD4^+/CD8^+水平高于对照组(P<0.05);观察组治疗后FIB、vWF、D-D、PAI-1低于对照组,tPA、FMD高于对照组(P<0.05)。结论:以注射用血塞通冻干粉针剂与尼可地尔联合治疗冠心病心绞痛,可提高患者心功能与血管内皮功能,改善患者心绞痛症状,预防血栓形成,改善免疫功能,疗效显著。
Objective:To analyze the effects of Xuesaitong freeze-dried powder injection combined with nicorandil on cardiac function and serum CD4^+/CD8^+ levels in patients with angina pectoris.Methods: From January 2016 to March 2018,98 patients with coronary heart disease and angina pectoris admitted to our hospital were randomly divided into the control group(n=49) and the observation group(n=49).The control group received routine comprehensive treatment.On the basis of this,the observation group was treated with Xuesaitong lyophilized powder injection combined with nicorandil.Both groups were treated for 2 courses.The clinical symptoms,ECG effect and cardiac function were compared between the two groups,heart discharge index(CI),stroke volume(SV),left ventricular ejection fraction(LVEF),atrial systolic mitral blood flow velocity ratio(E/A),cardiac output(CO),T cell subset(CD3^+,CD4^+/CD8^+),thrombus-related indicators [fibrinogen(FIB),von Willebrand factor(vWF),D-dimer(D-D),plasminogen inhibitor-1(PAI-1),tissue Type plas minogen activator(tPA)],brachial artery blood flow-mediated endothelium-dependent vasodilation(FMD) levels were counted.Results: The total effective rate of clinical symptoms and electrocardiogram in the observation group were 95.92% and 79.59%,respectively,which were higher than 77.55% and 57.14% of the control group(P<0.05).CI,LVEF,SV,CO,E/A after treatment in the observation group which was higher than before treatment and higher than the control group(P<0.05).The levels of CD3^+and CD4^+/CD8^+ in the observation group were higher than those before treatment and higher than that in the control group(P<0.05).FIB,vWF,D-D and PAI-1 in the observation group after treatment were lower than the control group,tPA and FMD were higher than the control group(P<0.05).Conclusion: Coronary heart disease with angina pectoris can be improved by combining Xuesaitong freeze-dried powder injection with nicorandil.It can improve the heart function and vascular endothelial function,improve the symptoms of angina pector
