热性惊厥附加症儿童的脑灰质形态学分析 被引量:4

摘要 目的:采用基于体素形态学测量(VBM)及基于表面形态学测量(SBM)方法,探讨热性惊厥附加症(FS+)儿童的脑灰质结构变化。方法:选取20例FS+患者及与之性别、年龄及受教育程度匹配的19名正常对照者进行磁共振3D T1结构像序列扫描,应用SPM12中VBM及CAT12工具箱进行全脑灰质体积、皮质厚度、脑沟深度、沟回指数及分形维度分析,采用独立样本t检验获得差异具有统计学意义的脑区。结果:与对照组相比,FS+组右侧额下回、右侧丘脑及左侧颞上回体积减小(P<0.05)。FS+组双侧颞上回、缘上回、左侧中央旁小叶、左侧额下回、左侧楔前叶、左侧顶下回、左侧颞下回、右侧顶上回及右侧梭状回皮质较对照组增厚(P<0.05)。脑沟深度、沟回指数及分形维度在FS+组与对照组间无统计学差异。结论:FS+患者存在多个脑区灰质体积减小及皮质厚度增加,其海马体积较正常组无明显变化。 Purpose:To analyze the changes of gray matter(GM)in children with fibrile seizure plus(FS+)using voxel-based morphometrey and surface-based morphology methods.Method:The GM volume,cortical thickness,sulcus depth,gyrificaiton index and fractal dimension on three-dimensional T1-weighted sequence of 20 patients with FS+were compared with that of 19 healthy subjects.Results:In patients with FS+,GM volume was reduced in the right thalamus,right superior frontal and left superior temporal gyrus(P<0.05).And they were also with significantly increased cortical thickness in both superior temporal,superior marginal,left paracentral,left inferior frontal,left precuneus,left inferior parietal,left inferior temporal,right superior parietal,and right fusiform gyrus(P<0.05).There was no significant difference in sulcus depth,gyrificaiton index and fractal dimension between these two groups.Conclusion:Increment of multiple regions of GM volume and cortical thickness were found in patients with FS+,and there was no significant difference in hippocampus between two groups.
