摘要
目的 比较各亚型注意缺陷多动障碍(ADHD)患儿的感觉统合能力,为临床制定合适的治疗方案提供依据。方法 实验组:将 108例符合DSM Ⅳ中ADHD诊断标准的儿童,根据整合视听连续执行测试(IVA CPT)结果,分为注意缺陷为主型、多动 冲动为主型、混合型 3个亚型。对照组为 108例性别、年龄与实验组相匹配的正常儿童。同时测试两组儿童的感觉统合能力。结果 与对照组相比,实验组儿童的大肌肉及平衡感因子分(37. 87±13. 47 )、本体感及身体协调因子分 ( 38. 82±12. 53 )显著降低 (P<0. 05),学习能力发展因子分 ( 31. 63±9. 31 )极显著降低 (P<0. 01 ),触觉防御及情绪因子分 ( 41. 78±10. 66)无显著差异 (P>0. 05)。与对照组相比,注意缺陷为主型儿童的大肌肉及平衡感因子分 (36. 88±11. 75)和学习能力发展因子分(32. 08±8. 66)极显著降低(P<0. 01);多动 冲动为主型儿童学习能力发展因子分(34. 77±11. 03)极显著降低(P<0. 01);混合型儿童本体感及身体协调因子分 (36. 13±12. 56)显著降低(P<0. 05),大肌肉及平衡感(35. 76±11. 87)和学习能力发展因子分 (28. 04±8. 23)极显著降低(P<0. 01),各亚型ADHD儿童触觉防御及情绪因子分与对照组相比差异不显著 (P>0. 05 )。结论ADHD儿童存在感觉统合失调障碍。
ObjectiveTo compare the sensory integration ability in children with ADHD subtypes and to offer accordance for clinical therapy .Methods Test group included 108 children diagnosed ADHD according to DSM-Ⅳ were divided into AD group,HD group and combination group by IVA-CPT. Control group included 108 healthy children who were matched with ADHD in age and sex. Sensory integration ability was assessed. Results Compared with control group, the scores of abilities of mass motor and balance and mal-coordination were lower in significance (P<0.05), The scores of abilities of learning decreased in more significant(P<0.01);there are no significance in the grade of touch excessive defense and emotional instability (P>0.05).In AD children, the scores of abilities of mass motor,balance and learning were lower in more significance(P<0.01).The grades of ability of learning in HD children were lower than healthy children(P<0.01).In combination subtype,the scores of abilities of mal-coordination were lower in significance(P<0.05),and the scores of abilities of mass motor,balance and learning were lower in more significance(P<0.01). There have no significant in the grade of touch excessive defense and emotional instability (P>0.05) in all subtypes. ConclusionADHD children have irregulation in sensory integration ability. Every subtype has different disorders in sensory integration ability.
