摘要
[目的]探讨中医医患会话的类型和心理交流机制,以构建医患会话的和谐模式。[方法]基于交互分析沟通理论,对中医医患会话语料进行分类并编制问卷,结合杰弗逊共情量表、成人依恋量表对大学生进行测试。[结果]互补沟通PC-CP型得分最高,为(23.28±4.76)分,是最佳中医医患会话模式;其次是AA-AA型,得分为(21.01±4.42)分;PP-PP型得分最低,为(11.59±4.63)分。冲突命令性用语越少,会话有效性越高。中医学生中,女生对AA-AA的评分高于男生;非医学生中,男生对AA-AA的评分高于女生。共情能力高的中医学生对PC-PC和AA-AA互补沟通有所偏好(r=0.22, P<0.05;r=0.27, P<0.01),低共情者对PP-PP和AA-CP有选择倾向(r=-0.47, P<0.01;r=-0.42, P<0.01)。有依赖倾向的非医学生表现为对除AA-AA以外的其他类型的选择偏好。[结论]中医医生应提高共情能力和沟通技能,根据不同病人调整会话类型,加强公众对中医药文化的认识。
Objective To explore the types and psychological communication mechanism of doctor-patient conversation in Traditional Chinese Medicine(TCM)so as to build a harmonious mode of doctor-patient conversation.Methods Based on transactional analysis and communicational theory,it used classification and questionnaire for doctor-patient conversation.The Jefferson Scale of Physician Empathy-Student Version and Adult Attachment Scale were combined to measure the students.Results The score of complementary communication PC-CP(23.28±4.76)was the best TCM doctor-patient conversation mode,followed by AA-AA(21.01±4.42)and PP-PP(11.59±4.63)was the worst.The fewer conflict imperative expressions the more effectiveness of conversation would be.Among TCM students,the score of AA-AA in girls was higher than that was in boys.Among non-medical students,the score of AA-AA in boyss was higher than that was in girls.Chinese medicine students with high empathy ability who had preference for PC-PC and AA-AA complementary communication(r=0.22,P<0.05;r=0.27,P<0.01),while those with low empathy ability who had preference for PP-PP and AA-CP(r=-0.47,P<0.01;r=-0.42,P<0.01).Non-medical students with dependency tendency showed preference for other types of selection except AA-AA.Conclusions Chinese medicine students should improve their empathy ability and communication skills,adjust their conversation types according to different patients,and enhance the public’s understanding of Chinese medicine culture.
