摘要 目的探讨重组人生长激素(rhGH)对特发性矮小症(ISS)的临床疗效及对患儿内分泌轴的影响,分析rhGH治疗ISS的机制。方法选择2017年1月至2019年1月于本院接受诊治的ISS患儿共74例,每晚于患儿睡前为其皮下注射rhGH 0.15 U/kg,共治疗6个月。比较患儿治疗前后的身高、体重、年生长速率(GV)、骨龄与总甲状腺素(T4)、促甲状腺激素(TSH)、胰岛素样生长因子1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP-3)水平。结果治疗前、治疗3个月、6个月,患儿骨龄、T4、TSH未出现明显变化,差异无统计学意义(P>0.05);与治疗前比较,患儿治疗3个月结束时与治疗6个月结束时的身高、体重、GV与IGF-1、IGFBP-3水平均明显增长,差异有统计学意义(P<0.05);与治疗3个月比较,患儿治疗6个月的身高、体重、GV与IGF-1、IGFBP-3水平均明显增长,差异有统计学意义(P<0.05)。结论rhGH对ISS的临床疗效显著,能有效促进患儿身高、体重与GV的增长,原因可能在于rhGH可有效提升患儿的生长激素-胰岛素样生长因子(GH-IGFs)轴功能。 Objective To explore the clinical efficacy of recombinant human growth hormone(rhGH)on idiopathic short stature(ISS)and its effect on the endocrine axis of children,and to analyze the mechanism of rhGH in the treatment of ISS.Methods A total of 74 children with ISS who were diagnosed and treated in our hospital from January 2017 to January 2019 were selected,and were subcutaneously injected with rhGH 0.15 U/kg for 6 months before going to bed every night.The levels of height,weight,annual growth velocity(GV),bone age and total thyroxine(Tyroxine,T4),thyroid stimulating hormone(TSH),insulin-like growth factor 1(IGF-1),insulin-like growth factor binding protein 3(IGFBP-3)before and after treatment like growth factor 1 were compared.Results There was no significant difference in bone age,T4,TSH before treatment,at 3 months and 6 months of treatment(P>0.05).The height,weight,GV and IGF-1,IGFBP-3 levels at 3 months and 6 months of treatment were higher than those before treatment(P<0.05).The height,weight,GV,IGF-1 and IGFBP-3 levels of children at the end of 6 months of treatment increased significantly compared with those at end of the 3 months of treatment(P<0.05).Conclusion rhGH has a significant clinical effect on ISS and can effectively promote children′s height,weight and GV.The reason may be that rhGH can effectively increase the growth hormone-insulin-like growth factors(GH-IGFs)axis function.
