摘要
目的:检测瘢痕疙瘩成纤维细胞中微小RNA-23b(miR-23b)和notch1蛋白的表达量,探究二者间表达关系及对瘢痕疙瘩的作用。方法:收集2017年12月至2019年11月在解放军第一四九医院就诊的瘢痕疙瘩患者130例,同时手术过程中取每位患者瘢痕组织及正常真皮组织。记录瘢痕部位大小,根据瘢痕疙瘩的严重程度将其分为轻度65例、中度43例及重度22例,对瘢痕疙瘩组织及正常真皮组织中成纤维细胞进行提取、培养,利用实时荧光定量PCR(qRT-PCR)技术检测成纤维细胞中miR-23b水平,利用免疫组化法检测notch1蛋白水平。利用Pearson法评价瘢痕疙瘩组织中miR-23b和notch1表达的相关性。结果:130例瘢痕疙瘩患者共有203个皮损,分别在胸部78(38.42%)个,肩背部59(29.07%)个,四肢40(19.71%)个,耳部13(6.40%)个,腹部8(3.94%)个,腰臀部5(2.46%)个。皮损累积面积为大瘢痕6例(4.61%),中瘢痕57例(43.85%),小瘢痕67例(51.54%)。与正常真皮组织相比,瘢痕疙瘩组织成纤维细胞miR-23b mRNA表达量和notch1阳性表达率明显增加,差异有统计学意义(66.15%vs 40.00%,P<0.05);miR-23b mRNA、notch1在瘢痕疙瘩不同临床分级之间表达差异有统计学意义(P<0.05),且随瘢痕疙瘩病情加重,miR-23b mRNA水平及notch1阳性表达率呈上升趋势;Pearson相关性分析结果显示,瘢痕疙瘩组织成纤维细胞中miR-23b和notch1呈正相关(r=0.529,P<0.05)。结论:miR-23b mRNA和notch1蛋白在瘢痕疙瘩组织中均呈高表达,二者呈正相关关系,可能同时参与瘢痕疙瘩的形成、发展过程。
Objective:To detect the expressions of microRNA-23b(miR-23b)and notch1 protein in keloid fibroblasts,and to explore the relationship between the expressions and their effects on keloid.Methods:From December 2017 to November 2019,130 patients with keloid were collected from the 149 Hospital of the PLA.During the operation,the scar tissues and normal dermal tissues of each patient were taken at the same time.The scar size was recorded,According to the severity of keloid,it was divided into mild 65 cases,moderate 43 cases and severe 22 cases.Firstly,fibroblasts from keloid and normal dermis were extracted and cultured,the level of miR-23b in fibroblasts was detected by real-time fluorescent quantitative PCR(qRT-PCR),and the level of notch1 protein was detected by immunohistochemistry.Pearson method was used to evaluate the correlation between miR-23b and notch1 expressions in keloid.Results:There were 203 skin lesions in 130 patients with keloids,78 on the chest(38.42%),59 on the shoulder and back(29.07%),40 on the limbs(19.71%),13 on the ears(6.40%),and 8 on the abdomen(3.94%),5 on the hip(2.46%).The cumulative area of skin lesions was 6 cases(4.61%)with large scars,57 cases(43.85%)with medium scars,and 67 cases(51.54%)with small scars.Compared with normal dermal tissues,mir-23b mRNA expression level and Notch1 positive expression rate in fibroblasts in keloid tissues were significantly increased,with statistically significant differences(66.15%vs 40.00%,P<0.05).The expressions of miR-23b mRNA and notch1 in different clinical grades of keloid were statistically significant(P<0.05),and with the aggravation of keloid,miR-23b mRNA level and notch1 positive expression rate increased;Pearson correlation analysis showed that miR-23b and Notch-1 were positively correlated in keloid tissue fibroblasts(r=0.529,P<0.05).Conclusions:MiR-23b mRNA and Notch-1 protein are highly expressed in keloid tissue,and they are positively correlated,which may be involved in the formation and development of keloid at the same time.
