摘要
目的:分析冠心病患者血清Vaspin和MCP-1水平变化及与患者心肌损伤程度及心功能的相关性。方法:将2017年7月至2019年9月我院收治的120例冠心病患者作为研究对象,按照疾病类型分为不稳定型心绞痛组(71例)、稳定型心绞痛组(49例),按照斑块稳定性分为不稳定性斑块组(53例)、稳定性斑块组(67例),按照病变支数分为单支病变组(57例)、双支病变组(38例)、三支病变组(25例),另将同期体检的94例健康者纳入对照组,采用酶联免疫吸附法检测血清Vaspin和MCP-1水平,比较两组血清Vaspin和MCP-1水平、心肌损伤标志物水平及心功能参数差异性,比较冠心病各亚组血清Vaspin和MCP-1水平差异性,分析血清Vaspin和MCP-1水平与冠心病患者心肌损伤指标及心功能参数的相关性。结果:冠心病组血清Vaspin水平显著低于对照组,血清MCP-1水平显著高于对照组,差异均有统计学意义(P<0.05)。不稳定型心绞痛组血清Vaspin水平显著低于稳定型心绞痛组(P<0.05),血清MCP-1水平显著高于稳定型心绞痛组(P<0.05);不稳定型斑块组血清Vaspin水平显著低于稳定性斑块组(P<0.05),血清MCP-1水平显著高于稳定性斑块组(P<0.05);三支病变组、双支病变组血清Vaspin水平显著低于单支病变组(P<0.05),血清MCP-1水平显著高于单支病变组(P<0.05),三支病变组血清Vaspin水平显著低于双支病变组(P<0.05),血清MCP-1水平显著高于双支病变组(P<0.05)。冠心病组血清心肌损伤标志物CK-MB、CK、H-FABP、cTnI水平均明显高于对照组,差异均有统计学意义(P<0.05)。冠心病组心功能参数LVEF、FS显著低于对照组,LVEDD、LVESD显著高于对照组,差异均有统计学意义(P<0.05)。相关性分析结果显示,血清Vaspin水平与LVEF、FS呈正相关(P<0.05),与CK-MB、CK、H-FABP、cTnI、LVEDD、LVESD呈负相关(P<0.05);血清MCP-1水平与CK-MB、CK、H-FABP、cTnI、LVEDD、LVESD呈正相关(P<0.05),与LVEF、FS呈负相关(P<0.05)。
Objective:To analyze the changes of serum Vaspin and MCP-1 levels in patients with coronary heart disease and their correlation with the degree of myocardial injury and cardiac function.Methods:120 patients with coronary heart disease admitted to our hospital from July 2017 to September 2019 were studied.According to the types of disease,they were divided into unstable angina group(71 cases)and stable angina group(49 cases).According to plaque stability,they were divided into unstable plaque group(53 cases)and stable plaque group(67 cases).According to the number of pathological changes,they were divided into single-vessel disease group(57 cases),double-vessel disease group(38 cases),and triple-vessel disease group(25 cases).In addition,94 healthy controls in the same period were included.The serum Vaspin and MCP-1 levels were detected by enzyme linked immunosorbent assay.The differences of serum Vaspin and MCP-1 levels,myocardial injury markers and cardiac function parameters between the two groups were compared.The differences of serum Vaspin and MCP-1 levels were compared.The correlation of serum Vaspin and MCP-1 levels with myocardial injury index and cardiac function parameters in patients with coronary heart disease was analyzed.Results:The serum Vaspin level of coronary heart disease group was significantly lower than that of the control group(P<0.05),and the MCP-1 level was significantly higher than that of the control group(P<0.05).The levels of CK-MB,CK,H-FABP and cTnI in the coronary heart disease group were significantly higher than those in the control group(P<0.05).LVEF and FS in coronary heart disease group were significantly lower than those in control group(P<0.05),LVEDD and LVESD were significantly higher than those in control group(P<0.05).The serum Vaspin level in the unstable angina group was significantly lower than that in the stable angina group(P<0.05),and the MCP-1 level was significantly higher than that in the stable angina group(P<0.05).The serum Vaspin level in the unstable plaque gr
