摘要
目的玻璃体切割联合眼内硅油填充术后患者需保持特殊体位且持续时间较长,因此体位管理尤为重要。方法选取2018年7月至2019年2月行玻璃体切割联合眼内硅油填充术患者,实施体位管理措施:(1)术后根据视网膜脱离范围及裂孔的部位采取不同体位的方法,面向下体位、半卧位、侧卧位、头低位等;(2)床头悬挂卧位标识牌,同时悬挂图文并茂的卧位健康教育卡片;(3)卧具的联合应用来减轻特殊体位带来的身体不适感;(4)眼科患者头部护理计划执行单的应用,保障患者正确的卧位姿势及并发症的观察。结果通过多种体位干预方式的联合应用,患者能遵医嘱保持正确卧位姿势,术后24 h达100%、48 h达98.75%、72 h达97.5%。术后3个月视网膜完全复位78眼,部分复位2眼,高眼压3眼,角膜内皮失代偿1眼,经积极治疗均恢复正常。结论通过对眼内硅油填充患者的综合性体位干预,对减少患者术后并发症,提高手术成功率,促进患者康复有着极其重要的作用。
Objective To investigate the application effects of comprehensive postural intervention in patients undergoing intraocular silicone oil filling.Methods The patients who underwent intraocular silicone oil filling after vitrectomy in our hospital from July 2018 to February 2019 were enrolled in the study.The position management measures were performed as follows:taking specific methods of different positions according to the retinal detachment’s scope and the location of the hole,including downword-facing position,semireclining position,lateral decubitus position,head down position,and it was necessary to hang the decubitus identity cards and the decutitus health education cards on the bedhead,and to use the bedding and linens to relieve the physical discomfort due to special position,moreover,to perform of the head nursing plan for ophthalmic patients to ensure the observation effects for patients’correct position and complications.Results After the combination application of mutiple posture intervention ways,the percentage of patients who could stay in correct position was 100%at 24 hours after operation,98.75%at 48 hours,and 97.5%at 72 hours,respectively.The retina of 78 eyes were completely reset at 3 month after surgery,and 2 cases of them were partially reset,there were 3 cases of ocular hypertension and 1 case of corneal endothelium decompensation,which were recovered to normal levels after active treatment.Conclusion The comprehensive position intervention through silicone oil tamponade plays an important role in reducing the postoperative complications,improving the operation success rate and promote the rehabilitation of patients.
