干预管理骨科Ⅰ类切口围术期预防性应用抗菌药物的效果分析 被引量:2

Effect of intervention on application of perioperative prophylactic antimicrobial agents in patients with orthopaedic Ⅰ class incision

摘要 目的探讨干预管理骨科Ⅰ类切口围术期预防性应用抗菌药物的效果。方法收集2018年1月至2018年12月山西省中西医结合医院收治的行骨科Ⅰ类切口择期手术患者87例设为对照组,另收集2019年1月至2019年12月实施药物细则管理后行骨科Ⅰ类切口择期手术患者83例设为干预组。应用调查问卷获取并比较两组应用抗菌药物种类、用药时限及不良反应情况。结果干预组术前0.5~2 h用药81例,占97.59%(81/83),高于对照组的87.36%(76/87),差异有统计学意义(P<0.05)。两组第1代头孢、第3代头孢药物使用率比较,差异未见统计学意义(P>0.05);干预组第2代头孢、克林霉素药物使用率高于对照组,氨基糖苷类药物使用率低于对照组,差异有统计学意义(P<0.05)。两组术后感染率、不良反应发生率比较,差异未见统计学意义(χ^2=0.210、0.502,P=0.335、0.274)。结论对围术期预防性抗菌药物进行细则管理,可把握术前用药时机、短程合理用药,不增加术后感染率。 Objective To investigate the effect of intervention on application of perioperative prophylactic antimicrobial agents in patients with orthopaedicⅠclass incision.Methods A total of 87 patients undergoing elective surgery with orthopaedicⅠclass incision admitted to Shanxi Provincial Hospital of Integrated Traditional Chinese and Western Medicine from January 2018 to December 2018 were collected as control group.In addition,83 patients who underwent elective surgery with orthopaedicⅠclass incision after the implementation of the drug regulations from January 2019 to December 2019 were collected as intervention group.A questionnaire was used to obtain and compare the types of antibacterial agents used,the time limit of medication and adverse reactions between the two groups.Results There were 81 cases in the intervention group who took medicine 0.5 hour to 2 hours before operation,accounting for 97.59%(81/83),which was higher than 87.36%(76/87)in the control group,the difference was significant(P<0.05).There were no significant differences in the use rates of the first-generation cephalosporins and the third-generation cephalosporins between the two groups(P>0.05).The second-generation cephalosporin and clindamycin use rates in the intervention group were higher than those in the control group,the use rate of class drugs was lower than the control group,the differences were significant(P<0.05).There were no significant differences in the postoperative infection rate and adverse reaction rate between the two groups(χ^2=0.210,0.502,P=0.335,0.274).Conclusions The detailed management of preventive antibacterial agents during perioperative period can grasp the timing of preoperative medication,and use the drugs reasonably in a short-term,without increasing the postoperative infection rate.
