摘要
目的促进临床安全用药。方法通过医院信息系统(HIS)选取干预前(2017年3月至12月)医院静脉用药调配中心(PIVAS)审核的112224组医嘱和干预后(2018年1月至10月)审核的230761组医嘱,对比干预前后不合理医嘱情况。参考最新版药品说明书及相关专业资料,对静脉用药的用药时限、储存条件、溶剂及用量、稀释溶剂、调配注意事项等信息进行梳理,并将更新的药品信息纳入PIVAS合理用药知识体系。结果干预前,共审核发现不合理医嘱647组(5.77‰),干预成功416组(64.30%);干预后,共审核发现不合理医嘱510组(2.21‰),干预成功446组(87.45%)。干预前后比较有显著差异(P<0.05)。结论通过整理静脉用药溶剂配伍注意事项,可发现临床用药中潜在的安全隐患,通过信息化手段干预,可促进临床静脉用药安全。
Objective To promote the safety of clinical medication. Methods A total of 112,224 sets of medical orders from march to December 2017 before the intervention and 230,761 sets of medical orders from January to October 2018 after the intervention reviewed by the Pharmacy Intravenous Admixture Services( PIVAS) were selected from the Hospital Information System( HIS). By referring to the latest version of the drug instructions and related professional materials,the information on the medication time limit,storage conditions,solvents and dosages,dilution solvents,and precautions for preparation of intravenous drugs were sorted out,and the updated drug information was imported into the PIVAS rational drug use knowledge system. Results Before the intervention,the hospital reviewed and found unreasonable medical orders of 647 sets( 5. 77‰) and 416 successful sets( 64. 30%);after the intervention,there were a total of510 sets( 2. 21‰) of unreasonable medical orders,and 446 sets( 87. 45%) were successfully intervened. The comparison before and after the intervention showed significant difference( P < 0. 05). Conclusion By sorting out the precautions for the compatibility of intravenous medication solvents,we can find the potential safety hazards in clinical medication,and the intervention of information technology can promote the safety of clinical intravenous medication.
