摘要
目的:观察解郁丸联合艾司西酞普兰治疗脑卒中后抑郁(PSD)的疗效,并初步探索其作用机制。方法:选取符合纳入标准的98例PSD患者,将其分为对照组47例(在常规治疗基础上使用艾司西酞普兰治疗)和研究组51例(在对照组基础上加用解郁丸治疗),1个疗程4周,两组均治疗2个疗程。比较两组治疗前后抑郁情况、神经功能评分、临床疗效,治疗前后血清核因子-κB(NF-κB)、5-羟色胺(5-HT)水平及微小RNA(miR-146、miR-221-3p)相对表达量;比较两组不良反应发生情况。结果:治疗后研究组汉密尔顿抑郁量表(HAMD)、美国国立卫生研究院卒中量表(NIHSS)评分低于对照组(P<0.05);研究组临床总有效率高于对照组(P<0.05);治疗后研究组血清NF-κB水平,miR-146、miR-221-3p相对表达量均低于对照组(P<0.05),血清5-HT水平高于对照组(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:解郁丸联合艾司西酞普兰治疗PSD效果确切,有助于改善神经功能、抑郁情况,其机制与调节血清单胺类神经递质、炎症因子水平相关,且安全可靠。
Objective:To observe the effect of Jieyu Wan combined with Escitalopram on post-stroke depression(PSD)and to explore its action mechanism.Methods:98 patients with PSD were randomly divided into the control group(n=47)and the study group(n=51).The control group was treated with conventional treatment plus Escitalopram;on which basis,the study group was also treated with Jieyu Wan.The treatment duration was eight weeks.Depression condition,neurological function,clinical efficacy,the serum levels of NF-κB and 5-HT,as well as the relative expressions of miR-146 and miR-221-3p were compared between the two groups before and after the treatment.The adverse reactions were compared between the two groups.Results:The HAMD and the NIHSS scores in the study group were lower than those in the control group after the treatment(P<0.05).The total clinical effective rate in the study group was higher than that in the control group(P<0.05).The serum level of NF-κB and relative expressions of miR-146 and miR-221-3p were lower in the study group than those in the control group after the treatment(P<0.05).The serum level of 5-HT was higher in the study group than that in the control group(P<0.05).There was no statstical difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion:Jieyu Wan combined with Escitalopram is effective in the treatment of PSD,and it can help improve neurological function,depression condition,which mechanism is associated with regulating serum monoamine neurotransmitters and inflammatory factors.The therapy is safe and reliable.
