摘要
目的:分析菖蒲郁金汤联合大剂量丙种球蛋白在小儿重症病毒性脑炎(Viral Encephalitis,VME)治疗中的临床效果。方法:将2018年1月-2019年4月于我院接受治疗的98例重症VME患儿纳为研究对象,并以双盲法将其分为西药组与中西药组,两组均为49例。西药组采用大剂量丙种球蛋白进行治疗,中西药组则在此基础上加用菖蒲郁金汤治疗。分析对比两组的临床症状、体征消失时间及中医证候积分,同时观察其血清神经功能指标[中枢神经特异性蛋白(S100-β蛋白)、神经元特异性烯醇化酶(Neuron-specific Enolase,NSE)、神经生长因子(Nerve Growth Factor,NGF)、人脑髓鞘碱性蛋白(Myelin Basic Protein,MBP)]水平及临床疗效。结果:用药后,中西药组患儿发热、咳嗽、意识障碍、肺部啰音及头晕呕吐等各项临床症状、体征消失时间均短于西药组;中西药组患儿各项中医证候积分均显著低于西药组;中西药组患儿NSE、NGF、MBP及S100-β蛋白水平均明显低于西药组;此外,中西药组患儿临床总有效率明显高于西药组(P<0.05)。结论:菖蒲郁金汤联合大剂量丙种球蛋白治疗小儿重症VME,可明显提高治疗效果,减少神经元损害及脑胶质细胞损伤,同时减轻中枢神经系统的实质性损害,从而改善患儿的临床症状及体征,促进其神经功能的恢复。
Objective:To analyze the effect of the Changpu Yujin decoction(菖蒲郁金汤)plus high dose gamma globulin on severe viral encephalitis in children.Methods:98 children were divided into the western medicine group and integrative medicine group by double blind method,49 cases in each.The western medicine group was given high dose gamma globulin.The integrative medicine group was treated with the Changpu Yujin decoction more.Results:After treatment,the disappear time of clinical symptoms and signs such as fever,cough,disturbance of consciousness,lung rumble,dizziness and vomiting in the integrative medicine group were all shorter than the western medicine group.The scores of TCM syndromes in the integrative medicine group were significantly lower than the western medicine group.The levels of NSE,NGF,MBP and S100-βin the integrative medicine group were significantly lower than the western medicine group.The total effective rate in the integrative medicine group was significantly higher than the western medicine group(P<0.05).Conclusion:The Changpu Yujin decoction plus high dose gamma globulin on severe VME in children can significantly improve effect,reduce neuronal damage and glial cell damage,and reduce the substantial damage of central nervous system,and improve the clinical symptoms and signs of children,promote the recovery of their neurological function.
