摘要
目的探讨冠心病病人同型半胱氨酸(Hcy)水平与D-二聚体(D-D)及凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、纤维蛋白(原)降解产物(FDP)等凝血指标之间的相关性。方法选择400例冠心病病人(冠心病组)及390名健康体检者(正常对照组),分别对全部观察者Hcy与各项凝血指标间的相关性进行Spearmen相关性检验;以Hcy<15μmol/L为水准分为两组,比较Hcy<15μmol/L组及Hcy≥15μmol/L组凝血指标水平。结果冠心病组病人FDP、PT、FIB、D-二聚体、Hcy水平明显高于正常对照组(P<0.05)。总体数据及冠心病不同水平Hcy组间凝血指标结果显示,Hcy≥15μmol/L组FDP、D-二聚体水平高于Hcy<15μmol/L组,差异有统计学意义(P<0.05);在两组整体数据中Hcy与D-二聚体、FDP、PT和FIB呈正相关(r值分别为0.274、0.161、0.228、0.125,P<0.01);冠心病病人Hcy与D-二聚体、FDP呈正相关(r值分别为0.248、0.146,P<0.01)。结论冠心病组Hcy与凝血指标明显高于正常对照组,Hcy及凝血功能指标可能对冠心病病人高凝状态具有协同作用;冠心病组Hcy与部分凝血指标、总体研究对象Hcy与大部分凝血指标呈正相关,提示Hcy≥15μmol/L组凝血指标水平明显高于Hcy<15μmol/L组,Hcy水平与凝血指标有相关性。Hcy与凝血功能的联合检测可以更准确地反映病人是否处于高凝状态,较单一指标更加可靠,在发病早期对冠心病的防止及治疗具有重要意义。
Objective To investigate the correlation between the levels of homocysteine(Hcy)and D-dimer,prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),fibrinogen degradation products(FDPs)in patients with coronary heart disease.Methods Four hundreds patients with coronary heart disease and 390 cases of healthy examinees were selected for the study.Spearmen correlation test was used to analyze the correlation of homocysteine and coagulation parameters between two groups.Coagulation levels of Hcy<15μmol/L group and Hcy≥15μmol/L group were compared,and the results were analyzed.Results The levels of FDP,PT,FIB,D-dimer and Hcy in patients with coronary heart disease were significantly higher than those in normal control group(P<0.05).The coronary heart disease that the levels of FDP,D-dimer in Hcy≥15μmol/L snb group were significantly higher than those in Hcy<15μmol/L snb group(P<0.05).The levels of Hcy and D-dimer,FDP,PT and FIB in the overall data of the two groups were positively correlated(r values were 0.274,0.161,0.228,0.125).Hcy in patients with coronary heart disease was positively correlated with D-dimer and FDP(r values were 0.248,0.146,respectively).Conclusion Hcy and coagulation indexes in coronary heart disease group were significantly higher than those in normal control group,which further confirmed that Hcy and coagulation function were synergistic effects of hypercoagulable state in patients with coronary heart disease;Hcy in coronary heart disease group was positively correlated with some coagulation indexes and Hcy in general samples was positively correlated with most coagulation indexes.These results suggested that the level of coagulation index in high Hcy group was significantly higher than that in normal homocysteine group,which also proved the close correlation between Hcy level and coagulation indexes.The combined testing of Hcy and coagulation indexes might more accurately reflect whether the patient was in hypercoagulable state,which was more reliable than a si
