摘要
目的:分析颈动脉内中膜厚度(CIMT)与同型半胱氨酸(Hcy)对急性冠脉综合征(ACS)的诊断价值。方法:依据病情类型,于我院治疗的140例冠心病患者被分为SAP组(70例)和ACS组(70例),另选同期健康体检者70例为健康对照组。比较各组CIMT、血清Hcy水平。分析CIMT、 Hcy与ACS的相关性及CIMT、Hcy对ACS的诊断价值。结果:与健康对照组比较,SAP组、ACS组CIMT [(0.44±0.08)mm比(1.17±0.17)mm比(1.50±0.19)mm]、血清Hcy水平[(10.21±1.11)μmol/L比(18.00±1.57)μmol/L比(22.00±1.57)μmol/L]均显著升高,且ACS组上述指标显著高于SAP组的(P均=0.001)。Spearman相关性分析显示,CIMT、Hcy与ACS均呈显著正相关(r=0.883、0.917,P均=0.001)。ROC曲线分析显示,CIMT、Hcy及二者联合诊断ACS的曲线下面积(AUC)分别为0.874、0.946、0.951,灵敏度分别为90.00%、88.57%、91.43%,特异度分别为85.71%、92.86%、91.43%,两项联合检测、Hcy单独检测的AUC均显著高于CIMT单独检测(P<0.05或<0.01)。结论:CIMT、Hcy联合检查对急性冠脉综合征具较高诊断价值。
Objective:To analyze diagnostic value of carotid intima-media thickness(CIMT) and homocysteine(Hcy) for acute coronary syndrome(ACS).Methods:According to disease type,a total of 140 patients with coronary heart disease treated in our hospital were divided into SAP group(n=70) and ACS group(n=70).Another 70 healthy volunteers were simultaneously regarded as healthy control group.CIMT and serum Hcy level were compared among all groups.Correlation among CIMT,Hcy and ACS and diagnostic value of CIMT and Hcy for ACS were analyzed.Results:Compared with healthy control group,there were significant rise in CIMT [(0.44±0.08) mm vs.(1.17±0.17)mm vs.(1.50±0.19)mm]and serum Hcy level [(10.21±1.11)μmol/L vs.(18.00± 1.57)μmol/L vs.(22.00±1.57)μmol/L]in SAP group and ACS group,and above indexes of ACS group were significantly higher than those of SAP group(P=0.001 all).Spearman correlation analysis indicated that CIMT and Hcy were significant positively correlated with ACS(r=0.883,0.917,P=0.001 both).ROC curve analysis indicated that area under curve(AUC) of CIMT,Hcy and combined detection for diagnosing ACS was 0.874,0.946 and 0.951 respectively;sensitivity was 90.00%,88.57%and 91.43%respectively;specificity was 85.71%,92.86%and 91.43%respectively;AUCs of combined detection and single Hcy detection were significantly higher than that of single CIMT detection(P0.05 or 0.01).Conclusion:Combined CIMT and Hcy detection possess higher diagnostic value for acute coronary syndrome.
