摘要
目的探讨乙型肝炎病毒(HBV)、丙型肝炎病毒(HCV)单独与双重感染者外周血淋巴细胞亚群及肝功能水平的差异。方法选取2017年1—12月辽宁省肿瘤医院收治的195例肝炎病毒感染患者为研究对象。根据病毒类型将其分为HBV感染组(HBV组,n=96)、HCV感染组(HCV组,n=85)与HBV+HCV双重感染组(双阳组,n=14)。将HBV组分为小三阳亚组[HBV表面抗原(HBsAg)+、HBV e抗体(HBeAb)+、HBV核心抗体(HBcAb)+,n=78]与大三阳亚组[HBsAg+、HBVe抗原(HBeAg)+、HBcAb+,n=18]。另选取同期体检健康者25例为健康组。采用流式细胞技术检测各组外周血淋巴细胞亚群百分比;采用日立7600全自动生化分析仪检测并比较各组丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、总胆红素(TBil)水平。结果大三阳亚组CD4+水平高于健康组、双阳组及HCV组,差异有统计学意义(P<0.05);小三阳亚组CD4+水平高于健康组,差异有统计学意义(P<0.05);大三阳亚组CD8+水平低于HCV组,差异有统计学意义(P<0.05);大三阳亚组CD4+/CD8+水平高于健康组与HCV组,差异有统计学意义(P<0.05)。HCV组的AST、ALT、ALP水平显著高于健康组,差异有统计学意义(P<0.05);HCV组的AST、ALP水平显著高于小三阳亚组,差异有统计学意义(P<0.05)。结论HBV、HCV感染均会导致机体淋巴细胞亚群紊乱,免疫功能失调,乙型肝炎大三阳者程度较重;双重感染不一定会加重肝细胞损伤与免疫紊乱程度;单独HCV感染会导致患者肝功能损伤,其损伤程度大于双重感染者。
Objective To investigate the differences of lymphocyte subsets and liver function levels in peripheral blood of hepatitis B virus(HBV)and hepatitis C virus(HCV)infected individuals.Methods A retrospective study was performed on 195 cases of patients infected with hepatitis virus who were admitted from January to December 2017.According to the virus type,patients were divided into the HBV infection group(HBV group,n=96),HCV infection group(HCV group,n=85),HBV and HCB double infection group(Dual infection group,n=14).The HBV group was subdivided into the Xiao Sanyang subgroup(HBsAg+,HBeAb+,HBcAb+,n=78)and Da Sanyang subgroup(HBsAg+,HBeAg+,HBcAb+,n=18).In addition,25 healthy volunteers who underwent healthy physical examination were divided into the healthy group.The flow cytometry was adopted to detect the percentages of lymphocyte subsets and liver function indexes such as alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP)and total bilirubin(TBil)were detected by Hitachi automatic biochemical analyzer.All the indicators were compared among the groups.Results The level of CD4+in Da Sanyang subgroup was significantly higher than that in healthy group,Dual infection group and HCV group,and the difference was statistically significant(P<0.05);the level of CD4+in the Xiao Sanyang subgroup was higher than that in the healthy group(P<0.05);the level of CD8+in the Da Sanyang subgroup was lower than that in the HCV group(P<0.05);the level of CD4+/CD8+in the Da Sanyang group is higher than that in the healthy group and HCV group(P<0.05).The levels of AST,ALT and ALP in the HCV group were significantly higher than those in the healthy group(P<0.05);the levels of AST,ALT and ALP in the HCV group were significantly higher than those in the Xiao Sanyang subgroup(P<0.05).Conclusion HCV infection can lead to disturbance of lymphocyte,immune dysfunction,the degree of triple positive is more serious,double infection may not aggravate the degree of liver cell damage and immune disorder;HCV infect
