摘要
目的了解广州市番禺区丙型肝炎(丙肝)患者丙肝病毒的基因型构成状况,分析丙肝病毒基因型与相关临床指标的相关性,为个体化抗病毒治疗方案提供参考依据。方法收集广州市番禺中心医院2016年共125例丙肝携带患者血清,采用PCR-反向斑点杂交法检测丙肝病毒基因型;同时检测血清丙肝病毒RNA(HCV-RNA)水平、肝功能水平及外周血T淋巴细胞亚群;另随机抽取15例健康人群体检标本作为T淋巴细胞亚群正常对照组。结果 125例患者检测出5种基因型,构成比为1b占48.0%、2a占8.0%、3a占10.4%,3b占6.4%,6a占27.2%;1b型病毒患者血清HCV-RNA载量、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBil)水平与非1b型比较,1b型病毒患者高于非1b型,差异均有统计学意义(P<0.05);1b型、非1b型病毒患者外周血CD3+CD4+、CD3+CD8+T淋巴细胞绝对值均低于正常对照组,且1b型低于非1b型,差异均有统计学意义(P<0.05)。结论番禺区丙肝患者感染以丙肝病毒lb型为主,其血清病毒载量、主要肝功能指标1b型高于非1b型,1b型外周血CD3+CD4+、CD3+CD8+T淋巴细胞绝对值低于非1b型及正常人群。
Objective To explore the genotype composition of hepatitis C virus（HCV） in patients with HCV infection in Panyu District of Guangzhou, analyze the correlation between viral genotype of HCV and related clinical indicators, and provide reference for individualized antiviral therapy regimen. Methods A total of 125 serum samples of patients with HCV infection who admitted to Guangzhou Panyu Central Hospital in 2016 were collected. The genotypes of HCV in serum were detected by PCR-reverse dot blot hybridization, while the serum HCV-RNA level, liver function index and peripheral blood T lymphocyte subgroups were also detected. Samples of 15 healthy individuals were selected as control group. Results Five genotypes were detected in 125 patients, type 1 b accounting for 48.0%, type 2 a accounting for8.0%, type 3 a accounting for 10.4%, type 3 b accounting for 6.4%, type 6 a accounting for 27.2%. Serum HCV-RNA load,alanine aminotransferase（ALT）, aspartate aminotransferase（AST） and total bilirubin（TBi L） level in patients with type 1 b virus were significantly higher than those in patients with non-1 b type virus（P0.05）. The absolute values of CD3＋CD4＋and CD3＋CD8＋T lymphocytes in peripheral blood of patients with type 1 b virus was lower than those of patients with non-1 b type virus, while those of patients with type 1 b and non-1 b type were both significantly lower than those in the normal control group（P0.05）. Conclusion In Panyu District of Guangzhou, the patients with HCV are mainly infected by type1 b virus. Their serum virus load and liver function index are both higher than non-1 b type. The absolute values of CD3＋CD4＋and CD3＋CD8＋T lymphocytes in peripheral blood of patients with type 1 b virus are lower than those in non-1 b type and healthy.
