摘要
目的 探究T细胞亚群、炎症指标及胎球蛋白A (fetuin-A)与慢性丙型肝炎的相关性。方法 回顾性分析 2019 年 8 月至 2020 年 10 月上海中医药大学附属曙光医院收治的 140 例慢性丙型肝炎(CHC)患者的临床资料。按照HCV RNA载量水平分为低载量组(31例)、中载量组(67例)和高载量组(42例),比较不同HCV-RNA病毒载量CHC患者的T细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8+)、炎症指标[白细胞介素-2 (IL-2)、白细胞介素-6 (IL-6)、白细胞介素-10 (IL-10)、白细胞介素-32 (IL-32)]和 fetuin-A 水平。采用 Pearson 相关性分析法分析 CHC 患者HCV-RNA病毒载量与T细胞亚群、炎症指标、fetuin-A水平的相关性。结果 低载量组、中载量组和高载量组患者的CD3^(+)水平分别(77.23±9.04)%、(76.18±8.71)%、(75.02±8.36)%,差异无统计学意义(P>0.05);低载量组、中载量组和高载量组患者的 CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平分别为(47.38±5.31)%、(30.23±4.42)%、(1.58±0.21) vs (42.12±4.27)%、(33.71±4.82)%、(1.27±0.17) vs (38.33±3.84)%、(36.26±5.46)%、(1.06±0.11),随着 HCV-RNA 病毒载量的不断增大,CHC患者的CD4^(+)、CD4^(+)/CD8^(+)水平明显降低,CD8^(+)水平明显升高,差异均有统计学意义(P<0.05);低载量组、中载量组和高载量组的IL-2、IL-6、IL-10、IL-32以及fetuin-A水平随着HCV-RNA病毒载量的不断增大,CHC患者的IL-2水平明显降低,IL-6、IL-10、IL-32以及fetuin-A水平明显升高,差异均有统计学意义(P<0.05);经Pearson 相关性分析结果显示:HCV-RNA载量与T细胞亚群明显相关,其中HCV-RNA载量与CD8^(+)水平呈正相关(r=0.472,P=0.027),与CD4^(+)、CD4^(+)/CD8^(+)水平均呈负相关(r=-0.505,-0.529,P=0.019,0.017);HCV-RNA载量与炎症指标、fetuin-A水平明显相关,其中 HCV-RNA 与 IL-6、IL-10、IL-32 以及 fetuin-A 水平均呈正相关(r=0.518、0.627、0.553,P=0.018、0.009、0.011),与IL-2水平呈负相关(r=-0.509,P=0.021)。结论 T细胞亚群、炎
Objective To explore the correlation between T cell subsets, inflammatory indexes, fetuin-A and chronic hepatitis C. Methods The clinical data of 140 patients with chronic hepatitis C (CHC) in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from August 2019 to October 2020 were analyzed retrospectively. According to HCV RNA load levels, the patients were divided into low load group (31 cases), medium load group (67 cases), and high load group (42 cases). T cell subsets (CD3^(+), CD4^(+), CD8^(+), CD4^(+)/CD8^(+)), inflammatory indexes[interleukin-2 (IL-2), interleukin-6 (IL-6), interleukin-10 (IL-10), Interleukin-32 (IL-32)], and human fetal globulin(fetuin-A) of CHC patients with different HCV-RNA load were compared. Pearson correlation analysis was used to analyze the correlation between HCV-RNA viral load and T cell subsets, inflammatory indexes, fetuin-A level in patients with CHC. Results The levels of CD3^(+)in low load group, medium load group, and high load group were (77.23±9.04)%, (76.18±8.71)%, (75.02±8.36)%, and the difference was not statistically significant (P>0.05). The levels of CD4^(+),CD8^(+), CD4^(+)/CD8^(+)were (47.38±5.31)%, (30.23±4.42)%, (1.58±0.21) in low load group, (42.12±4.27)%, (33.71±4.82)%,(1.27±0.17) in medium load group, (38.33±3.84)%, (36.26±5.46)%, (1.06±0.11) in high load group, with the increasing of HCV-RNA viral load, the levels of CD4^(+), CD4^(+)/CD8^(+)in patients with CHC were significantly decreased, and the level of CD8^(+)was significantly increased, with statistically significant differences (P<0.05). The levels of IL-2, IL-6, IL-10,IL-32 and fetuin-A in low load group, medium load group and high load group were significantly increased with the viral load of HCV-RNA;the levels of IL-2 significantly decreased in CHC patients, while the levels of IL-6, IL-10,IL-32 and fetuin-A significantly increased, with statistically significant differences (P<0.05).
